Don't apologise, all very helpful. The above 11mmol thing I dont remember from my course so I either forgot or it was added later as I did mine in 2007. Would make sense. I'm one of those people that hates even mild hypos so tends to err on the side of caution.
You're right about night testing, haven't done it for a while. Will try and do it sometime this week.

In your particular case you should not of corrected at lunch because you already had insulin working away at this time, at its peak near enough................you only correct if you have no QA insulin active, ie...4-5 hours after last injection............

Resisting snacks is hard, I know from personal experience............I think the most important point for you would be consistency, if you have this snack every day, you can spot patterns and adjust your insulin for it......

When I mentioned carb free I was purely referring to basal insulin testing, it isn't permanent, just sometimes necessary when testing background insulin, but there are many things you can have that are carb free, meats, cheese, salad.........

I would do the same tonight, have your dinner as normal, then nothing till the next morning, set an alarm for 3 am, record your BG at that time.............this will tell you if your going low at night, resulting in the high...........you can repeat this test to confirm..............if all is well overnight you need more BI in the evening........

DAFNE states that 1 unit of insulin drops the blood sugar level by 2-3mmol/l....................but it also states that above 11mmol/l this observation may not apply............this is the case for me.......and in my personal opinion, for everyone else to varying degrees.............so the higher you are the more you need to bring you down.................if I was 18, I would take 13 units to bring me down to 5.5, so 1 unit brings me down 1mmol/l.........

below 10 - 1 unit drops 2.5mmol/l
10-13 - 1 unit drops 2mmol/l
13-17 - 1 unit drops 1.5mmol/l
above 17 - 1 unit drops 1mmol/l

Everyone is different, I worked mine out by assuming 1 unit drops me by 2.5 at all levels, then, provided the carb count is right, recorded how much 1 unit actually dropped me by at certain levels.........

It took time, but was worth it.............I would recommend a similar investigation for yourself............

DAFNE does provide flexibility, but a lesson I have learned over the years is that you need to fine tune those principals for yourself, and sometimes the control you want just cant be achieved without dramatic changes to diet, I have been cutting my daily carb load, reluctantly, but its giving me the results I want............sometimes DAFNE cannot provide all the answers........

Why shouldn't I correct at lunchtime if still high? Because its too close to the snack? I must admit I struggle to resist the warm bacon rolls when they come around and I've had an early breakfast, this is something I can cut out. However eating carb free is very difficult whan you work in town and rely on Pret/Eat/Subway etc!
My main concern is when I'm high getting my BG down seems difficult. This morning I woke up with a BG of 18!! I had gone to bed with a BG of 9. So I may have gone low overnight, but I adjusted for it with a +6. At 1pm my BG was still 10.
I feel I've lost control. I know if I went back to fixed meals/times it would be easier, but thats the whole reson I went on DAFNE - to cope with my work/lifestyle.

Have been controlling this by a little lie to myself...whenever I see 23:55 as a time I know that I was late to bed.

Thanks for reminding me about this.
I did discover the diary modification option a while ago but had got used to my existing system and so have not used it.

Eh?

I use 8mm needles and regularly use my arms, however I wouldn't describe it as the upper arm, but the subcutaneous tissue above the tricep muscle........is this the area you are referring to......?

And if so, what is the significance of needle depth and this location....?

i have changed to 4mm needles too and think they are great. Also, you can inject into your upper arms with them which you are not meant to do with anything above 4mm. This is much more convenient sometimes i find.

Any blood test result after midnight goes on to the chart for the next day.

I have just discovered a setting that allows you to change the time your diary day ends. So no more split charts!

This is brilliant for me!

1/ go to your diary.
2/ scroll to the bottom then click "click here to edit your diary settings" this is in blue.
3/ A heading of "blood glucose diary settings" will come up.
4/ Look for "diary day ends at"
5/ Use the input box to add your new time.

I can use this as well when I am abroad and have to allow for the time difference.
There are other settings here which you can change as well.

If you know this already then you're my granny and here are the eggs.

I think Nicola Sturgeon closed the waiting lists a few years ago which i was mad about (even though i don think i want a pump) but it seems now they are open again? I cant see them becoming available any time soon though Sad

They are planning on providing them to children first but will make more available to adults over time. The east of Scotland (well anywhere not in the Greater Glasgow area!) seems to have more pumps available (prior to this announcement). There are only 21 pumps, all allocated to children - which is great for them, not so much for us over 18. Hopefully this announcement will make them adjust their estimated figures for the next few years! - http://www.scotland.gov.uk/Publications/2010/08/17095311/10

Either way I feel a round of letter writing coming on, if only to get them to open the waiting list so I can get on it!

They are available in Scotland, its just its very hard to secure funding, a fact that our Consultants know too well and are therefore reluctant to put anyone forward for one....................there are many in Scotland with one though........

And its a postcode lottery...............I know of an individual in Dundee that got one without any trouble, was offered one really, without having to prove anything, and then there are some in my PCT who have considered writing to politicians and the like.......

If they are finally making the process easier and consistent that's great news, where did you read/hear about this change............?

The news today that, over the next couple of years, pumps may become available to adult diabetics in Scotland was one that brought me great joy. However, how can I even dream of getting one of these life changing devices if the waiting lists are closed, and have been for some time?

My first observation is your lows in the afternoon, this seems to be happening because you are eating at about 10-11, injecting for it, then having lunch a few hours later, but you have corrected at the lunch time, when you shouldn't have, also your carb amounts are quite big also, which makes it harder to control..................the less carbs, the easier it is........

Its hard to spot a specific problem but I would be tempted, if I was you, to test the daytime BI dose with some carb free meals. Just to see if that dose can help get the numbers down before you start on your insulin/carb ratios........

your overnight BI dose seems OK, but again, testing is the best way to find out..........

Well the diabetes Centre have written to a diabetes Nutritionist they have who knows DAFNE and are awaiting response. In the meantime I'm still struggling so I've booked a GP appt to get me referred to Maidstone.
I've put my last weeks figures down. I'm on Novorapid and Levemir. As you can see I'm struggling.

Hi batoutofhull

I adjust by 1 or 2 units at lunchtime as i usually go to the gym about 1 1/2 hours after lunch. But it depends on the lunchtime reading. if in double figurs i will take insulin according to the amount of carbs in my lunch meal, if my reading is under 10 then i will adjust down one or two units of insulin Smile

Hi and welcome to the forum..............

Its hard to pin point any specific problems as there isn't much information to go on however I have made some observations.

You seem to be waking up high most mornings, this must be exhausting. This can be down to the dawn phenomenon, not enough background insulin, or not enough quick acting insulin. On a few nights though you had your evening meal about 19:00 or 20:00, tested before bed, the level of which was not far off the next morning, you would assume your BI dose is correct then but your QA from dinner has not finished working yet, so I think your 18 BI at night should increase a few units.....

So just follow the normal drill, carb free, 3am tests, no QA including corrections, until you wake up the same level as you went to bed on......

The first thing I always look for in someones diary is there background dose overnight, as if there are no signs it is doing the job then trying to adjust doses in other areas is pointless.........

How long have been graduated........?

I usually gym & swim about 3x/week, and tend to go straight after lunch. This is a regime I have been doing for about 4 years, and certainly started gymming before doing Dafne. I won't say I'm hooked, but feel great on the drive home [mission accomplished!] and sleep well too!
Despite cutting my QA to half for my lunch carbs, when I returned home post-gym/swim I was often testing at hypo level. Over the last few months I haven't been bothering taking any QA with lunch if I am going to the gym. My testing post-gym/swim have been slightly better, and just occasionally have been into double figures. But these tend to be more of a one off. I just want to minimise the times I'm testing in the hypo range, and my body not giving me the hypo signals. Confused
I suppose it all depends on how hard you exercise, and working out what works for your own body. We are all different!

I go to the gym three times a week and test myself before and after exercise. I also went to weight watchers and lost a stone within three months. the two combined ie exercise and diet seem to work, but no slacking is allowed! Very Happy

Wow thats amazing well done, my hba1c not particularly high but have had a lot of highs and lows recently, hoping they settle down over next few months and that by next year which will be a year on for me I will feel as positive as you.
