Yes I'm realising now there's no set rule to rounding up/down. By the way, a packet of Walkers cheese and onion crisps is classed by them as 12.5g and I'm counting as 1 CP. So if I have a sandwich which I know is 3 plus crisps equals 4 then with my ratio that makes 6 units.. If I classed crisps as 1.5 CP, 4.5 x 1.5 = 6.75 then that makes 7 units. So presumably to those of you who are not as resistant to me as insulin, one unit more or less makes a lot of difference? But t hen as you say you go by your pre meal test and then decide what insulin to take.

Thanks for replies. I am really happy with my meter even though it may not be the most all singing all dancing one. So can I ring up Accu-check and say "can I have a new meter please?" and they should give me one even though there is nothing wrong with it? Why do they sell them in chemists, who buys them?

Its not good to be adjusting BI all the time, but the harshness of life is that our BI needs fluctuate all the time......

So I wouldn't discard the basal testing just yet, but work with your team, they can cater for your specific needs.

What about your waking on high levels, has this been sussed out yet, and if so, what the plan to tackle it?

I think some folk can just be naturally sensitive to insulin, or it may just be that you are normal and I am abnormal, that sounds more likely............... Laughing

I have 4 correction factors, you may have seen me outline them in other threads.......basically the higher my BG is, the more I need..........fortunately I only ever use 2 of these on a regular basis, 1u/2.5mmol/l if below 10, and 1u/2mmol/l if between 13-15mmol/l. And if I am bad and over 17, then 1 unit drops me by only 1 mmol/l. For you though, these would definitely not work due to your increased sensitivity.....

If your not going to be moving your BI, the only way to tackle the frequently occurring lows is QA, and its seems that you cant really go much lower, you could push it down a wee bit further maybe..................if not, its time for the pump........... Wink

Oh behave! Razz I'm not sure how active I am considering I don't go do any sports on a regular basis. I just do stuff like walk to work every day and that gives me about 30 mins exercise in total most days... walk to most places if I can. You know, nothing really stands out to me... but perhaps I'm more active than I realise? :/ My boyfriend does a run once a week and cycles to work these days... he thinks I'm more active than him but I can't see how if I'm honest.

My DAFNE team think that I should not adjust my BI doses anymore so I haven't done. My last carb free (lunch) was 25th (have added it to show).

Golly I hope I don't have too many different correctional factors. >_< These hypos are a frequent occurance... I will have another look at time of the month stuff just to rule that out too...

Rising at 7 or close to that time would suggest dawn phenomenon due to their being only a few hours between the 3am test and waking.......

I wouldn't of though that you would need to adjust so much because of your food filled weekend either......how extreme was it? The liver releases a steady a stream of glucose which wouldn't change dramatically just because you were eating lots. Your resistance to insulin may have been upped a few notches if your eating habits over the weekend were not controlled well and high sugars followed, that may be the explanation..........

If Lantus works better then I totally agree. You would expect folk on little amounts of basal, and who are relatively sensitive to work well with Lantus, me however, I need lots, 64 units in total, 28 in the morning, 36 at night....... Smile

If your really sensitive and are experiencing hypos because of it, a pump is in order..............has this been mentioned/offered. How many hypos do you have in a week?

Sorry about the mid meal confusion there, it can be quite hard to get back in the zone after a few days break from the thread.....

No concrete conclusions on your background behavior yet?

My correctional patterns go by blood sugar level, but most go by insulin resistance, which makes sense as well, its perfectly possible that at different parts of the day you need different correction factors............

try and record how much 1 unit drops you by at different times, obviously ensuring the carb count/ratio/background is spot on first is needed......I done this, but I noticed a pattern with the BG level after reading it again in the handbook.....

Ah, the mid meal comment was in response to your previous post (sorry for taking so long to reply to it!).

Yes, I find last night and yesterday afternoon quite remarkable. I am wondering if my correctional amount changes after my evening BI injection... but by how much I am not sure. So hard to keep track of all this stuff lol. Completely puzzled by the afternoon low the day before....

I wouldn't say skin and bone - enjoy food too much to be that skinny! Very Happy I'm your typical 5'5" 10 stone odd gal.

Hi NRB,

I usually get up around 07.00 but this varies according to my shifts at work.

I think that you're right and the problem definitely lies with my BI - I just wouldn't have thought that I would have needed to make this level of adjustment to it after 24 hours of "naughtiness". Oh well, that will teach me.

I did try Levemir for a period of about 5 months but it didn't suit me at all. I'm just one of those people who suit Lantus better. I know that we're in the minority but I have found a fair few others.

And, in answer to your post on my other thread, yes I am incredibly sensitive to insulin which I guess isn't necessarily a bad thing after 24 years of diabetes but it does make for a fair amount of yo-yo-ing with my sugar levels which can make life harder!

Thanks for the advice - much appreciated as always.

Whats your thoughts on the hypos last night? seems strange, you corrected with 2.5 units for a 17.9 and went low, I would need 12 units to get me back on target............its amazing how different we are, well, its quite easy to explain, I am a good few stone over weight, and you are probably skin and bone........ Wink

What mid meal spikes specifically are you concerned with........

also remember to get BG readings when you are eating/injecting, they are most useful and are greater priority than the mid meal ones.......... Smile

MelissaF, I'm a sensitive half unit pen using diabetic too! With the half unit pen, it makes such a difference with this sort of thing.

On that note, does anyone know if there's a Levemir half unit pen? It's come up every now and then in my mind... I use a humalog half unit pen for meals/corrections.

9.9 to 4.4 to 5.8... I'd love results like that on a more frequent basis!

Not sure what to say about the mid meal peaks though. I know they can be very unpredictable. Sad

Here's my most recent tests... getting a bit slack again with multiple injections in the evening.. but I those hypos.. I just don't get them at all. Sad This used to happen when I was adjusting my ratios.

Emailed my health team too.. shall see what they suggest.

My average daily numbers [breakfast, lunch, dinner bed] over the last month and a half are 12.3/7.3/6/8.8

Mine is 8.8/9.4/8.9/8.7......................these have been the best results I have had in a long time............I have never been below 8.0 since starting on DAFNE........ Sad ............

The averages given there are a bit too good though, you would need to be on a pump to achieve them....... Wink

What time do you normally get up at?

The first impressions would be Dawn Phenomenon, normal levels at 3 am, high when waking, back down once fuel and insulin for breakfast has been given.......

I think more background testing is needed, I asked if you had done any in the other thread.... Wink

You really need to NOT correct before bed when basal testing so as to eliminate the QA variable, then always remember the 3 am as well......if your really high, correct and do the test another night....

I couldn't say for sure but I wouldn't of expected you to go low after a 7.1 at 3 am, but its not impossible.....

So just go through the steps methodically, establish what you BI dose is doing overnight, adjust accordingly.....then move on to the daytime, with carb free or missed meals....

Unfortunate with Lantus there is no flexibility for varying BI needs from day to night. You can split the dose though, its just personally I feel its no use and Levemir would be better, I am bias though........

What is interesting is that this has all happened after lots of carbs over the weekend, which is not the usual behavior. So this reaction may only be a temporary one and you may find your BI needs return to normal soon.

In the meantime, get testing....... Very Happy

I personally would work out the CPS in 1 gram, so the 100g carb value divided by 100.

Then weigh the powder and multiply that value by the 1g CPs........

From the ASDA nutritional web page it states that for the strawberry one that there is 67.3g of carbs per 100g of powder........therefore 67.3/100 = 0.673g of carbs in 1g of powder.......

Weigh the powder your going to use and multiply the weighed value by 0.673, this will give you the amount of carbs in grams for the powder you are going to use..........

How much milk goes in? I wouldn't imagine it would be lots and therefore you would only be looking at 1CP [10g] of carb likely........

Its says on the ASDA page that an 89g portion prepared with semi skimmed milk is 11.5g [1CP], but how is that possible if the box only holds 59g. For this reason alone I would choose to work out the CPs in 1g route as described above.......

I assume you have scales, if not, invest in some............eventually you will remember how much insulin to take for a typical Angel delight and you wont need to do all this work.......

The worked example be meltow is exactly how I would do it, multiple by your ratio then round up or down......

Depending on your preference, it could be a good idea to consider you pre meal test when deciding to go up or down.......I personally just use the normal rounding up/down rules regardless of what my BG is at.....

I am quite happy to have my BG at 4.5 - 5 mmol/l......... Wink

Its always good to get some fresh meters every couple of years I think.............

Just so you are keeping up with the times, and using the strips and meters that are most cost effective.......not a lot of people would probably think about that side of it, to be honest, I don't most of the time..... Wink

As they are keen to sell a volume of test strips to the NHS most meter manufacturers will supply you a meter free.
Meter models change at a pace. If you have had good service from AccuCheck why not continue with them.
There are some that cost, with gizmos that you are not likely to need. They are in the minority - usually only the very latest developments.
Personally I use an Abbott Medisense Optium Xceed. Current meter is around three years old. Have a look at their website through the link - scrolling the first section at the top shows what they currently have available.

Plenty more to choose from though. You may find that your GP has a preference for the cheapest test strips, which is OK if they are for a decent meter.....but there are some poor quality products out there too and they tend to use cheap test strips.
