Hi both,

This is fine in this forum Smile

I thought the algorithms were the same but will check it out and let you know. Thanks for calling it out.

Topic: Site Development / pdf diary downloads

Hi Andy,

If there are over 10 entries in a day contained in the diary being generated the PDF will default to landscape orientation to fit them in horizontally. Below 10 it defaults to portrait which will give around 5 days per page. Unfortunately there is a tradeoff here between more days per page whilst keeping the text size readable.

I can't guarantee I can get more per page but will certainly try!

Topic: Site Development / Injection site list

Hi all,

As a temp fix you could enter the BI as a separate entry when taken with QA then enter the new injection site, it might be better than cluttering the results further?

Just my 2 pence worth.

Pizza and Pasta will almost definitely give you some delay, pizza the most, so this would likely be the culprit fo ryour high morning readings......

Do you have a 80g -90g meal every night? Nothing wrong with this really, but the reality is, we all eat too much carbohydrate and it doesn't always agree with us all.........

I personally find after 90-100g of CPs blood sugars become a bit trickier to manage. I am sure it was from a John Walsh book on insulin that said if you half your weight in pounds, this will be the amount of grams of carbs you can eat and the normal insulin dosing rules will apply...anything above and you need to think about more insulin than you would normally give....so may not apply to you, but paints a picture......

I dropped my carbs for about a week and a half and my BG was amazing....... Wink

you should have pizza again and see what it does to BG and how you managed, you will probably find its quite hard to manage, so should be kept for special occasions...... Razz

Don't divulge anything you don't want to, just saying you are having problems is enough to know it can be effecting your levels........

If you feel comfortable, I would definitely think about moving to 9pm or later even............this does run the risk of highs in the evening due to BI running out but it can be compensated for. I find I cant really have a carb free dinner as I need some, even just a little, QA to tide me over till my next BI dose at 10pm..........

Its a real pain in the ass to be honest......cant have carb free breakfast either as i need CP and QA to switch off my liver in the morning......

Its times like these that a pump would come in real handy, but because I am managing OK funding for one would be a miracle, and that's if I didn't live in Scotland.........

Keep us informed of your changes and results, and try for some carb free lunches and dinners, so you can see what is really going on with the daytime Levemir..........

My high carb meals are in the evening about 8 or 9, pizza/pasta/potatoes etc... (not all at the same time Very Happy )

Low carb meal alternatives when being good will be meat/fish with salad and possibly potatoes/bread, but 2CP not 10CP to 20CP as naughty meals.

When do you normally eat? How many carbs is in your low carb meals?

Its quite common for high fat and I suppose high carbs meals [150g +] to have a delayed absorption, but you would still expect it all to be processed by the time the morning comes.....

If you don't get higher readings when you don't have high fat/carb meals then its unlikely its the DP your observing, even though there will be some liver action happening as it does with us all........

Hi novorapidboi26,

Thanks for the info, I have my Levemir split 12 hours apart 11am and 11pm with a bigger dose at night, I may try leaving the night one a bit later until I got to bed.

I do 3 (have carbs early) during the week, I have noticed when I'm on a diet and cutting out or having low carb meals in the evening I don't get DP, its to do with a heavy meal overnight I think.

Thanks

Phil

Topic: Questions for HCPs / Dawn Phenomenon possibility?

Hi NB,

Thanks for the reply.

Some time ago (at least 6months) I changed my eve BI from 9pm to 7pm... and have been thinking about putting it back again now, especially after our chat here and some discussion with my partner. I think the reason I put it back to 7pm was because of highs in the evening, but it was such a long time ago, I forget exactly.

I'm unsure how much detail I'm happy to go into here about my problems... It's a lot of different things and unfortunately, my control does seem to be affected. My DAFNE team are aware but tell me to keep correcting with QA for any random highs. When you feel like you're fighting a losing battle, you do get a bit down... I'm not on any depression medication if you're wondering; I'm managing to cope without.

I've gotta go for now...

My averages are considerably different also, I thought maybe it was because the app was working out an average based on the results currently stored on the phone, however I have the last 10 days stored, but the 7 day average is about 0.3 mmol/l out..........

Topic: General Discussion / Dawn Phenomenon

H guys,

I suffer from it, and actually everyone experiences it, its just some it is more noticeable........

I do 3 things to give me the best chance of waking in single figures......

These are:

1. Have a split dose background insulin and ensure my evening dose is at its maximum, so no lows at 3 am, just steady as she goes...

2. Instead of a 12 hour split for the BI insulin I take my evening dose closer to bed time, so 15 hours after my morning dose, Levemir last 12-18 hours, so still within limits. This gives me more active units of Levemir in my system at 4-5am, when the DP begins.

3. Have carbs and insulin as early as possible, which means getting up early, which for most is achievable because of work, however if you don't work, you still need to get up. This is because giving yourself fuel and insulin will tell the liver to stop its release of glucose into the blood stream.

So Nicky, a split dose is what is needed if your suffering from DP and plan to fight it, the split allows you to focus separately on the morning levels and on the daytime background requirement. Lantus can be split, but the effects of dose adjustment with Lantus takes time to settle, so in my non professional opinion, its no use for the majority of diabetics.......

Phil, I would expect good results for you if you adopt a similar regime to the one I described above.........

Do you have a 12 hour split on your doses?
When do you normally wake up, and do you have breakfast upon waking......?

Topic: Questions for HCPs / Dawn Phenomenon possibility?

Hey,

I think you still need to tweak your evening BI, you are still waking high but seem to be dropping from your 3 am tests, so this suggests dawn phenomenon, however there are not many 3am test results along side pre bed tests with no corrections, this would confirm what your 22 dose is doing, it should be holding you steady, that's all, if its not, then its wrong..........I also think you should take your evening BI before bed, this may mean compensating with more BI during the day and/or more novorapid to make up for the missing BI at dinner time.........

its also strange as sometimes your correction just don't seem enough, eg 1 1uit for a 16.3, but then you take 2.5 for a 17.3 and you go low, very strange, although the low may have been down to the BI being too much...........

its all about the BI still I'm afraid, there is just no sound evidence yet on what its actually doing..........especially during the day, you need to go carb free......starting with lunch, as you need carbs for breakfast for the DP.........

I see you have been 'down' and 'anxious'! Do you feel this has been effecting your blood sugars, is everything OK?

Topic: DAFNE Online Mobile / BG Average Algorithm

Hi,

I assume this is a question for Simon, but posting for others to see / answer / chip in.

I have noticed that the BG average on the website is giving a 7 day average of 9.4 ( I know this is not good Sad ), but the iPhone app is giving 9.7. Are the maths not the same on the iPhone or is this due to the iPhone also giving break/lunch/dinner/bed averages too, rounding error from using the 4 averages to work out the total average?

The 4 separate break/lunch/dinner/bed averages might be nice on the website for none iPhone users, it helps show when in the day I'm having issues, suppose on the the graphs show this though.

Its not an issue, just wondered? As a developer I tend to spot these things, it makes me feel better when there are wrinkles in other sites too Very Happy

Topic: Carbohydrate Counting / new to carb counting

Hi and welcome to the site................

Getting to grips with carb counting is fairly straight forward fortunately, its getting that right alongside dose changes etc that can be tricky............

The first thing to do is test your background insulin dose, firstly overnight, and then during the day..........you do this by omitting carbs and insulin for a period of 5 hours roughly, so miss a meal or go carb free, this will let you know if its right or wrong, allowing you to adjust it up or down depending on the outcome. Then and only then can you start to think about insulin/carb ratios with your novorapid........

A discussion with your team would be advisable.......

your results don't have any noticeable pattern, which can be expected, but its clear your putting in the effort, so well done........

Hi simon . Works fine now thank you.

Something I did notice with the pdf is that it only shows 2 days worth of results per page. Is it possible to make it 7 ?

Just a suggestion

Thanks andy

Hi Stew,

You won't have to delete them, you might have to go into one of the entries and save it again that will trigger the upload again.

Topic: Carbohydrate Counting / new to carb counting

Hi jonopesda,

Welcome!

Firstly, please, please make sure you always have a bottle of Lucozade with you up on the roof, and when you are going up and down the ladder.

Secondly, I wonder if your ratios are right, particularly at lunch and dinner.

Thirdly, perhaps you should talk to your diabetes team about whether you are on the best basal insulin. Possibly a split dose of Levemir might be better.

Do remember to change only one thing at a time, and by small amounts. But check with your team first.

Best of luck!

Roger

I got a similar message a few minutes ago when I transfered my DAFNE diary entries for the last couple of days to the website using my iPhone 4s. I carried on regardless, so there are about 10 entries sitting on the phone which don't appear in the on-line diary. Assuming that being patient (not a strong point of mine) works, will the records upload or will I have to delete them from the iphone and enter them again?

Update on current progress following on from previous post... Starting to feel again that BI adjustments... be it times or doses adjustements... may be in order still. Planning to put Morning BI up again from 11 to 13... but want to check with DAFNE team and offer here the chance to comment as well.

Saw the hosp doctor on friday... unsure what to advise for me, insulin wise. Frustrated.
Thanks in advance for any helpful responses.
