I'm having them all over the place really ie on waking, during the day/evening & in the early hours. The worst ones have been if I've woken early or in the early hours, especially if it's had to be an assisted one with my hubby ie the whole of my body is twitching and jerking = not very pleasant at all Evil or Very Mad !!!

What would be the first thing you'd look at doing?
I've decreased my basal/lantus first from 42u to 36u then from 36u to 32u, I only did the latter on Tuesday 20/11/2012.
Since Tuesday I've also started to bring the time of my dose earlier in the evening from11:00pm - done it by an hour each evening.

Also wondering if a bit of supper ie a sandwich might be an answer to get me right through the night.

Humalog/QA - before meals - carb counting
Lantus/BI - evening

Hypos

I'm currently having lots of hypos ie on waking, during the day/evening, during the night.

Things I've changed and when:
1 - BI dose decreased/Tuesday 20/11/2012 - on advice from my DSN
2. - Evening meal ratio changed from 1:10 to 1.5:10/Tuesday 20/11/2012 - ditto.

Would it be too many changes if I also started to take my Humalog right after eating a meal?
I normally take it 15 mins before to let it start working then just on the very odd occasion have taken it after.

Read this in the leaflet
"Humalog is a rapid-acting insulin. Take Humalog within fifteen minutes before eating or right after eating a meal".

I'm normally up at about 07:30am - 08:00am ish

I could possibly take it at 8pm with dinner but I thought you had to make the time earlier by just an hour each night.

But if it was you how would you mark this all down in your dafne diary ie having your BI earlier then also doing a test at bedtime?

If your last insulin dose of the day is BI (background insulin) and you take this early evening but you don't go to bed until much later would you

- put the BI amount into your diary and the time
then
- test at your actual bedtime ie when you get into bed and mark this as BED

I normally take my lantus (BI) at 11:00pm or an hour or so either side. This is usually also my bedtime.
I take humalog (QA) at around 08:00pm ish when I have my evening meal.
I'm going to start taking my lantus earlier in the evening. Any votes on my new time please?

Ahh I see lol Laughing .

Anyway It's no too late to reduce it again tonight (after already reducing last night) by another 2u so from 34u to 32u (in total from 36u to 32u). I'm also going to start taking it much earlier in the evening. I usually take it about 11:00pm or within an hour either side. Then I'm going to wait 4/5 days / 1 week to see what changes it brings.

I did actually phone my DSN & she confirmed together with me all of the above. DSN also said I could maybe even reduce it to 30u.

How many units of insulin would you have on a
0.5:1 ratio

for example 5 CPs / being 52.0g of carbs?

would it be 0.5 times 5 = 2.5?

Then with a one unit pen (I don't have a half unit pen) & taking into consideration bs levels being low or high it would be rounded down to 2u or rounded up to 3.

I've never done DAFNE so am I right with my workings out please?

I'm having hypos all over the place ie waking up to them plus having them during the day.
One week ago on Monday evening 12/11/2012 I reduced my lantus (BI) by 6u so from 42u to 36u (on a say so from my DSN). I decreased it again last night by 2u so from 36u to 34u. I can't help thinking I should've maybe reduced it by 4u. I know lantus should be left for 3 / 4 days / 1 week before making any more changes. Is it too late after just 1 night be reducing it once more then stick with this change for the next few days?

I count carbs & take humalog before bkfast, lunch & dinner.
Bfast & lunch ratios are 1:10 Dinner ratio is 1.5:10

I'm currently having 36u lantus at bedtime.

My pen only allows me to dial up in 1 units not half units.

you have probably been seeing people refer to such ratios on diabetessupport, however these people will be on pumps most likely. There will be some who are injecting and this may work for them.........

I am sure I read somewhere that the standard/average carbs processes from 1 unit is 10 for adults and 15 for children, this may now be out of date but DAFNE is based on this..... Very Happy




That's correct I've never done DAFNE Sad . My one way of learning / teaching myself is by way of the forum. Now thanks to you lovely people on the forum I'm getting to know all about the dose adjusting - thank you Very Happy !

Yes I've seen people refering to such ratios on diabetessupport. Another good forum.

Whatever amount of CPs you have, you just multiply that by your ratio (i.e. 1.5 or 2 or 2.5 or 3) for example if you were on 1.5:1 and you had 8 CPs that would be 8 x 1.5 = 12 units. If you were on 2:1 with 8 CPS you would have 8 x 2 = 16 units. So it all depends on the ratio you are on.

If you're still not sure then I would strongly suggest you talk to your own diabetic team who will be able to suggest the right ratio for you based on your readings.

No it's not for a correction. It's for my teatime dose. I've been doing 1u QA of insilin for every 10g (10CPs) carbs so if I've had 40g (4CPs) carbs then I would inject 4u QA. I'm now doing (for my new dosage) 4u QA plus an extra 2u QA so 6u QA so what would a ratio of 6u be for 40g (4CPs)?

It's re the Clickstar pen wihich is reuseable, takes 3ml cartridges.

I've seen mention that you can control the insilin delivery speed. How do you control the insulin delivery speed on this pen?

I currently use the blue Autopen 24 for Lantus 3ml cartridges.

If your ratio was one unit of insulin to every ten grams of cabs (1:10) then had to add on an extra two units of insulin what insulin to carb (i:c) ratio would this be please?

Also would like to know this for marking down as a diary entry.
