Really messed up here.
Have lunch at 2:30pm - 3:00pm then tea at 7:00pm - 7:30pm however the day has gone on and am just having lunch/tea whatever you want to call it. So I'm missing a meal & QA injection. Take BI at 10:00pm. I'm off out this evening also but not for a meal just to see a gig.
BS was 12.8 which I presume is because I've not eaten since breakfast. Had QA dose for meal and a correction.

What would you do in this situation?

Pilau rice (the packet type - Asda) - 51.4g CHO
Mince pie - 38.0g CHO
Total CHO - 51.4 + 38.0 = 89.4

How would you enter this as CP's in the BG diary?

Also just a quick check please, how many units on a 1:10 (one unit to every 10g CHO?)

It could be seasonal changes that are contributing. I thought BI would go up in winter though due to your body working harder behind the scenes to keep warm. But its not working out that way for me. The good thing is your insulin levels are dropping, it may seem scary, but in the long term its good.......

Has your daytime levels been effected with the reduction of BI?



I've also not had any changes with food ie still eating the same things/amounts. Also no increased activity.

Daytime levels don't seem to have been effected.

Maybe like you say it could be seasonal changes.

I might decrease mine by just another 2u either tonight or tomorrow night.

Had to increase my lantus/BI in October due to lots of high (teens/20's/30's) BS levels.
During November have had to keep decreasing due to lots of hypos - how much longer can this carry on? At this rate I'll be taking no BI - frightening.com!

On Saturday I went out for an Indian and ate the meal about 08:10pm ish then It was 01:10am - 17.2 before bed. I didn't correct for a fear of going hypo.
On Sunday morning it was 07:56am - 13.7. I did a correction of 2u and no food (which is why I wanted to know how to enter a correction dose with no food)
then 09:24am - 2.9 - had 5 jelly babies
then 10:39am - 6.4 - bfast

The only time I ever do a correction dose is with a meal or if I had a high BS first thing in the morning.
I would never do a correction before bed.

This morning I had a high BS in the teens and did a correction of 2u QA. Jelly babies were at the ready Laughing .

My question is how would I enter a 2u correction without a meal time dose in my diary?

BI/lantus last reduced on Friday evening by 2u from 32u to 30u so it's been three days so I'm going to reduce it tonight by maybe 4u.

Ps - I know there are some things missing from my diary so please ask away for any further info.

Thank you very much! But I can only try & do it with help from people like yourself who have been lucky enough to do DAFNE.

I come on to the forum to ask my questions because I get more help here than I do at a 10 minute/half hour appt with my DSN. No disrespect to her because when I do see her she's very good.

I like to get involved with the forum by asking my questions and by lurking around to see what others are saying/doing about things. I wish there had been something like this 20 years ago when I was dx.

To you nrb26 and to all other DAFNE people on here thank you for all your help.

1.5:10 ratio

for example 2 CPs / being 20g of carbs?

would it be 1.5 times 2 = 3
so 3u QA

What is the 3 day rule?

I know they say to make changes to lantus every three or four days or a week, but due to me having night hypos or waking to them I'm going to try reducing it each night. So if I have a night hypo or wake in the low 4s I'll reduce it again the next night until I'm OK.

What would you do in my situation:
1. do as I'm going to do
or
2. leave it for every three or four days or a week
or
3. do it a bit sooner than three/four days/1 week

There is no doubt that timing of your BI, even when on a single dose can be experimented with, for example some folk take there dose in the morning for whatever reasons. For you however it looks like the dose it too much so I would keep your dose time the same until you get a morning BG within 1-2mmol/l of your pre bed test for more than 2 days in a row.....



Re liquid being quicker that's because your not having to chew something for ages.

Re put in the carb amounts of the hypo treatment - would this be the same way as I enter for meals? Supposing I'd had 20g or 30g carbs?

I think that the lows your are experiencing are down to dose amount as opposed to timing. With a split dose of Lantus and with Levemir you can consider changing the times of dose to earlier/later in the evening.....but with a single dose, It wont make much difference I would imagine...

I usually treat hypos with 4 x jelly babies or 4 x fruit pastilles then when BS is back up again if at home 1 x round bread with a smearing of jam just to give the bread some taste.
Yesterday though what happened no jelly babies eek so I had water with 3 x tspns of sugar which seemed to work a lot quicker. I HATE dextrose tablets!

Thanks for mentioning about the "dose amount as opposed to timing".

Thanks for confirming re the timing of injections/dose changes.

The reasons for wanting to inject after the meal is because I thought maybe it might slow things down a bit and help to prevent hypos but then again maybe not. I've not been getting hypos in between meals. Please don't give me any more hypos to contend with lol Laughing

"If your Lantus dose is still in question then you shouldn't be changing your QA dose....

That's how I would look at it anyway....

DAFNE teaches to change only one thing at a time in order to keep as little variable in the equation as possible, and that your background dose should be spot on before any meaningful changes are made to QA doses......

So you have decreased your Lantus to stop the hypos coming, yet you have increased your pre dinner insulin ratio.......

One may be cancelling out the other......"


This is exactly what I thought ie one thing at a time.

I know it's my lantus/BI at fault, I'm having to much of it. But now I don't know where the ++++ I'm upto also do I keep on with the decreasing of the lantus/BI and maybe alter the teatime/dinnr dose back to 1:10 instead of 1.5:10?? very Confused !
