Topic: Questions for HCPs / BG rise at end of Lantus Hi, I think it is a case of Lantus running out, but like others I'm not a HCP so its just personal opinion. Given the scenario you originally described your QA should have stopped working ( 6 hours after lunch, it generally should only last 4-5 hours) and you then need your BI to keep your levels steady. If it has also reduced in effectiveness ( it should 'tail' off gradually between 12 and 24 hours according to its profile) your BG will start to rise. The only sure way is to take your BI and not eat anything all day, if your BI is right then your BG should stay at similiar levels. This is not an easy thing to do I know but that is the theory anyway. Although slighting your BI dose does mean you have two lots of BI acting at the same time, they should again in theory be reducing and increasing in tandem and so not be acting totally at the same time.I think most of us do find a split dose better, but as ever we are all different. If your control is quite good now it may be you can see more effect if you split your dose. It may be worth a try however it doesn't sound like your consultant would support it, so you really need a DAFNE educator to support you otherwsise its going to be hard to do on yuor own. |

Topic: Questions for HCPs / Blood Glucose range whilst pregnant Hi, Thanks for your reply. You dont happen to know what the bg should be before bed do you? As you can see from my readings they are a little up and down... not sure if the ups are an effect from the lows, however I have adjusted accordingly. I have seen the diabetic doctor at the hospital when I first became aware I was pregnant and he adjusted both QA and BI insulins. After numerous hypos I have decreased ratios and BIs where possible. I spoke to him again last Monday but was not happy to increase evening ratios. My BG was raising late evening with no QA in my system only BI, however still found I was experiencing night time hypos.. Strange.. but no one can explain this, not even him. I am trying desparately to keep my levels under 10 and eat a balanced diet. Im due to have a dafne course review next week... perhaps they will have some answers :-/ Thanks caroline |

Topic: Questions for HCPs / Blood Glucose range whilst pregnant hiThis is actually mentioned in this month's Balance mag - apparently 3.5 to 5.9 pre meal and no more than 7.8 after meals. very tight! (actually sounds totally unatainable to me, but is seemingly possible so the docs say). I guess that's why we need so much extra input from the health service while pregnant. Are you seeing people regularly? |

I actually split it not long after I did Dafne about 3 years ago and didn't notice a great deal of improvement, although I think it's probably safe to say that my overall levels then were under less control than now. Maybe I'll be able to identify more of an improvement if I were to try it now. Funnily enough, once my consultant was aware that I had split it he was quite horrified and suggested that having 2 sets of BI and QA all working at the same time made identifying problems much harder - think he probably had a point in that respect. Then again, he'd not been on the dafne training....!Thanks again |

On the subject of the Spanish Civil War, after a birdwatching trip to the Spanish Pyrenees and the Zaragossa area (including the ruined town of Belchite) last June, I thought I should know more, so got a copy of Anthony Beevor's "The Battle for Spain". I slogged through its near 600 pages - a bit of a struggle but great preparation for Orwell's "Homage to Catalonia", which I thoroughly recommend (along with Laurie Lee's "As I Walked Out One Midsummer Morning and its sequels). Topic: General Discussion / What are you reading? Just finished Connelly's Nine Dragons Stew B. Been a while since I last read one of his novels and realised that I do really enjoy a Harry Bosch yarn. Hope to pick up The Reversal soon.Initially found the web site mentioned useful to complete the A to Z....or however far it goes....list of Sue Grafton novels. Currently contemplating Hugh Thomas - The Spanish Civil War - a mighty tome......well it has made it out of the wardrobe bottom to the bedside drawers anyway. Regards Garry |

I would agree with the rest, split the Lantus, I am on Levemir but I find easty times to remember is breakfast and dinner (6:00am and 5:30pm)........that might not suit you but as long as you get coverage for the whole day....... |

Topic: Questions for HCPs / BG rise at end of Lantus I had a similar situation before completing the DAFNE course, and it was suggested to me to split my Lantus dose into two.After the DAFNE course I went back to one dose of Lantus, but changed the time to when I have an evening meal, though at times it seems that the Lantus occasionally seems to run out before the evening injection. If you decide to change the time of the Lantus injection don't change it in one go, change the time gradually (1 Hour a day). When it was suggested to me to split the dose I was advised to divide it in two and have the two injections 12 Hours apart (i.e.11:00AM and 11:00PM) though it is difficult to remember to do the injection mid morning depending on what time you normally do the injection before bed. |

Topic: Questions for HCPs / Information on menstrual cycle How has everyone been getting on? Ive managed to almost sort out the hormonal time bit, but I think that once Ive had the initial days of the period, my insulin demand seems drop as I come up to the time where I take the pill again - resulting in time shifts with hypos (it was hypos after dinner earlier and the last 4-5 days up to pill has been lunchtime hypos - today was at 12noon, "great fun"). Pre pill seems to be 8 or 9-ish morning and 9 night for BI, but yet again Ive ran out of time because first pill is tonight! I plan to take 7 tomorrow and I bet my body will decide it wants more insulin again with the pill taking (ARGH! Make your mind up! Lol!). Shall see what happens!![]() |

Its worth factoring in what insulin has caused the hypo in the first place rather than generalising. I guess that the quick drops of BG are caused by too much QA and slow drops are by BI - but this is only a common sense based assumption. The trick of DAFNE is to work out which I suppose. |

Topic: Carbohydrate Counting / Beans and lentils Further to this, has anyone noticed that on american forums they talk about counting just about everything - so if they have a tomato salad they would count the carbs in the tomatoes and take insulin for them - this seems to be against the whole idea of "free" foods as taught in DAFNE, but I've found it really helps. For instance, if I have a meal now that's made up entirely of free foods such as veg/salad I take a unit or so to cover this and have found that it works. I suppose if you eat anything in quantity it's going to have an effect upon BG... |
