Topic: General Discussion / BI back down? Hi - I agree with your own daignosis the BI needs to be reduced. Try and follow a step by step dafne appraoch - I know how hard it can be when your whole world feels upside down with either lots of highs or in your case at the moment lows.![]() Start by Lowering your BI - when you get your overnights sorted then start looking at your ratio's in the daytime one by one as I think you are right in saying that things for you will get a bit higher - so they may need to be adjusted - rather than having to correct all the time. I find that i am really sensitive to insulin and raise or lower ratio's by only a 1/4 each time, but my advice would be to try and work on one at a time - as sometimes trying to do too much at once can just cloud what's going on. I too am on Lantus, but take it on a split dose taking it at 10.30 am 5u and 10.30pm 7u - but I am still jugging with it all and have just been going through what you are describing and am still adjusting - have reduced nightime BI and concentrating on getting my teatime ratio right - so I can go to bed on a good level, before I move on and look at my lunchtime ratio. It really is trial and error and it can take its toll on you trying to get it right. Stick with your plan and beieve in yourself and if your still struggling seek advice from your HCP. Good luck. |

Topic: DAFNE Online Mobile / Android app Hi All,I graduated from DAFNE in Melbourne yesterday, and I'd also de interested in an Android app, not that I've seen much of the iPhone one. I agree with AllanR, I've been using OnTrack and putting in the totals (for CPs, QAs & BIs) then writing the adjustment breakdown info in the notes section. I guess it is a stop-gap solution though. - John |

Topic: General Discussion / Advice please I have moved the eve BI to just before bed, the last meal I eat is at 7pm at the latest because I cant sleep if I eat late,I will keep recording the results but Im thinking a pump is the next step |

Topic: General Discussion / Some advice please Well, at lunchtime (1.23pm) was 4.3. Had a 0 cp meal and by dinner (7pm) BG was 7.6. Had 1.5 cp for dinner and took 6 units QA (4:1 ratio). Just checked BS just now (9.45pm) and bs is at 10.7.Looks like I will have to go on a different BI or ??. Have a dafne review on the 6th of this month so hopefully I can be sorted. |

Topic: General Discussion / Advice please If its the dawn phenomenon, then you cant really stop it, it will always happen, i think the drug metformin can stop glucose dumps from the liver but i couldnt comment on its effectiveness................moving your BI dose to immediately before bed may help as you will then have more units in your system at the 4, 5 am time...............i moved mine from 1800 to 2230 and can wake with single figures now..............but trying to get the daytime dose to last longer or compensating with QA in the evening may be needed...... that could be an option for you if your sure the dose is correct...... getting up earlier to carch the rise in BG is another option...........i get up at 630, so my body starts the hormonal response about 430-530, this is probably down to my internal body clock being used to the 630 wake, if i got up an hour earlier the response may even start ealier, so you might never be able to catch it effectively........ some think that eating slow released carbohydrate before bed results in you still digesting food/absorbing carbs in the early hours that essentially tells the liver that the glucose dump is not required, and so dropping the BG slightly, there will still be increased insulin resistnce though............. theres lots of ways to try and trick the liver but sometimes this is not enoug and pump is needed, but you will need to prove this with results obviously...... |

Topic: General Discussion / Changes to the NHS I am very concerned about all the changes the government are proposing to make and the speed of the reforms. I sent a list of questions I had to my MP, but she never answered any of them, she just said they were a good thing. I wrote back and she replied that she would pass them on to Andrew Lansley (the Health Secretary). Eventually, I got a letter from Earl Howe (remember him?) He didn't answer any of my questions either; instead, he referred me to the 'Listening Exercise'. I didn't even know there was such a thing - has anybody else heard about it?If anybody's interested the website address is www.dh.gov.uk/healthandcare Its name doesn't actually give any clues that it is a Listening Excercise, does it? It smacks of a political ploy, so that they can say that they listened, before implementing the changes. This 'Listening Exercise' closes at the end of May, so if you want to have your say, you'd better do it quick. |

Topic: General Discussion / Advice please I have been using levamir as BI for a while now and I have got the dose right for the day but try as I might I cant lower the early morning levels. My BG is normal at 3am but at breakfast its more like 8,9,more sometimes. I know about dawn effect and after breakfast it lowes to near normal. I have altered the eve BI dose to no effect any other ideas please??? |

Topic: Carbohydrate Counting / Jacket Potato It seems a baked potato actually absorbs faster than pasta, but about the same as rice.................and white bread, so starchy, but still high medium to high GI, which means higher spikes as opposed to a slow gradual impact on BG................try it without cheese and see what happens.......the impace may be less severe, then you can start thinking about other toppings............. |

I would just like to say how glad I am to have been able to attend it, I learnt so much from Kaylee and Caroline and meet some nice people too (Diabetics). Topic: General Discussion / Some advice please Its definetly something to discuss with your team.....but they may just offer another background insulin that has a longer duration..................if you can prove with results how long it is actually lasting that would help.........See how the higher dose of 24 goes, I suspect you may need to still go further, also the more units you put in, the longer its active duration, they may have told you this in the course...........so theoretically you could end up on 30 units and it may also last past the time when the hormones kick in........... Keep up th good work........... ![]() |

How weird!I do remember my DAFNE team saying about how it's cooked effects it's out come... didn't click in my head that it was like pasta and rice though (probably because most potato doesn't change due to water, but in a way, for a jacket to behave differently to say chips, does make some sense when you think about it). |

Topic: General Discussion / Some advice please Not very good BS this morning at 11.2. Bedtime was at 11.7, had 1 cp and 4+7 QA along with my 22 BI.That is me tried the 22 BI for 2 days so will try 24 tonight. With regards to a cp free bedtime snack. I used to always have a cp free snack and it was only because the HC told me that was why I was going high. With no carbs the brain was telling the liver to pump out glucose as it needed it. Withy my BI being isophane and the duration being 8-14 hrs and I suspect it only lasts 89 hrs, would I better taking it 3x daily? 1 in the morning 8am another 1 at 4 or 5pm and then at bedtime. I know there is a risk of them overlapping but the morning one has ran out by 5 or 6pm. If it lasted 14 hrs for me 2 a day would be fine. When first put on isophane, 15 yrs ago, was told take it at dinnertime which is ok if it lasted the full duration. However, if it only lasted 10 hrs that would explain why I was always high in the morning. Take it at 5pm, 10 hrs later at 3am it has ran out so nothing to cover the dawn phenomenon which, incidentally I was never told about. Thanks for all the help your giving me it is very much appreciated. Best regards Derek ps they told me the metformin stops the liver producing glucose. |

Topic: Carbohydrate Counting / Jacket Potato Hi guys,Interesting discussion and strangely enough (contrary to what health professionals previously believed / understood / advised), jacket potatoes have a relatively fast absorption rate, so the CPs get into your blood stream pretty quickly. We know from experience that different cooking methods (eg oven baked; microwaved; cooked in foil) appear to make a difference to the CP value of a potato once it's cooked, most likely due to how much water is lost in the cooking process, however the 'validated' nutritional text books give the 3CPs per 100g value which is what's stated in the DAFNE CP List. Many DAFNE Educators suggest, where possible, to weigh the potato while it's raw, to get a more accurate value (based on 1.5 CPs per 100g) then it doesn't matter how it's cooked. The other thing to consider is whether or not you eat the skin..... you may lose up to 1CP if you leave the skin ![]() Hope this helps the discussion? Carolin |
