As C says, if there is no QA isulin orcarbs having an effect then it must be the BI insulin that needs tested and adjusted.........

the dawn phenomenon may also be contributing, having some carb and insulin is sometimes needed to stop the liver dumping glucose....

i would also suggest a 3 am test, as you say you had ben gven the hypo scenario by your mother, so you should have set out to confirm whether this was the case........

Hi David,
I would do a 3am test (I know this may be unsuitable but maybe at the weekend?) as that will show what your liver and body is doing as there will be no QA insulin in your system at that point and will give you an indication on whether the amount of Lantus your taking before bed is correct.
I tend to agree with your mum and the increases in morning BGs suggest that you may have been having hypos through the night.

Need some help please. I take a split dose of Lantus, 10 units before bed and 10 units first thing in the morning. My BG in the mornings have been usually 8 with the occasional 9, so just above target. To try and get these readings down I increased my bedtime lantus from 10 units to 12 about a fortnight ago, but then found my morning BGs had jumped to 14, 15 and 17s. My mum suggested I could be having hypos during my sleep. So a week later I reduced my lantus dose from 12 down to 11 and have now found my morning BGs to be around 10, 11 but most often 12s. I'm completely baffled by this, does anyone have any thoughts/suggestions? Any response greatly appreciated.

The release of ‘fight or flight’ hormones such as adrenaline make us insulin resistant.
So strenuous exercise can have a marked effect on BG post exercise.

Thanks Garry, that makes perfect sense. When on my course I was including corrections but when I queried it I was told to best leave it out. With corrections included and the extra QA during illness, in my case it would keep my BG at a more comfortable level.

Hi, thanks for reply.
Well, fingers crossed the increase has worked. With a morning BG of 5.7 this morning, I'm hoping my increase of BI last night continues this morning trend...I'll monitor for a few days, do some 3am checks and come back with the results.

I have always included corrections Gordon P as I find that as sickness comes on, I am making progressive corrections and if I bring in sick rules I want to get the situation under control quickly. So I always include corrections.

Only from another graduate like yourself, but I would agree.
We all need recognise how long changes in Lantus dose take to have any effect. Seems to take 3 days for most of us.
My DAFNE consultant recommends 10 to 20% changes in BI to see quick outcome - so I think you are right Smile

Mike, if it goes up but you havent had any breakfast or any quick acting insulin then i'd be looking at your slow acting (basal) insulin. However if it goes up after breakfast (which it should a little) then maybe you need to check you've counted your carbs correctly.

My BG reading is perfect at breakfast but by lunch has gone very high. The same thing happens if I eat breakfast or not. Which insulin should I increase?! Any help very gratefully taken......

This is the most bizarre thing ever - glad I'm not the only one!
I've recently started to go to the gym in the morning (30 minute run before breakfast); I'm usually 6-8 when I wake, and end up ~15 an hour after. So I do a few units (1 unit takes me down 2 mmol) to correct, as well as my breakfast units, but it continues to rise! Sometimes up to 17 two hours after.
The idea of injecting before is scary, but think I'm going to have to try it. Will let you know!
What's more bizarre is that when I was diagnosed over 10 years ago, I was always told to eat something (at that stage, a mini mars bar a day Confused !) before PE classes, to stop me going low! Is that just because I used to do 2 mixed injections per day rather than DAFNE, or how did I not notice I was always high.
Very odd, but so glad I found this!

Hi, could anyone confirm that when Calculating TDD, that includes all insulin taken in that day including corrections. Speaking and researching this is giving me conflicting information on wether to include corrections or not. Any help much appreciated.....

Hi, as far as I am aware Ketones are caused by the abscence of insulin to transport blood sugar into cells resulting in 'fat' being used for energy. Therefore a carb free meal cannot result in ketones, it can result in a hypo if you BI is too high which is exactly what you are trying to establish. If you do have a carb free meal to test your BI its a good idea to test a bit more regularly just to ensure what IS happening to your BG.

I find 3 jelly babies works a treat followed by some toast.
Helps avoid over-compensating.
I also find 3 dextrose tablets is enough (for me anyway).

As mentioned on the DAFNE course, a small carton of Apple or Orange is about 2CP, perfect amount for a hypo, more bulky than Dextrose to carry round though.

I use one or two 25g tubes of Gluco Gel, (persciption) or a few Gluco Tabs followed by toast to back it up. The Gel tastes bad, but it works well.

The only thing to watch out is I've heard of people that often have carb free meals can develop ketones so just a wee minder Smile
