Topic: General Discussion / NHS Medication Changes: Are you affected?

I got a letter today from my GP! Exciting stuff lol! Very Happy

NHS said:
The doctors are currently reviewing all repeat medications. The aim of this is to ensure that all medications prescribed are both cost effective and appropriate in line with up to date evidence based medicine and economic policy.

It has been decided to change your novofine needles, as current guidelines have shown that the new needles are equally or more effective when used as directed. Your new prescription will be for:

Omnican fine needles which are a universal fit.

If you have any concerns regarding this change please feel free to contact the surgery and make an appointment with the doctor who they will be happy to discuss the changes with you.

Interesting how they send me a letter about this but not the other change (the blood testing machine and strips). :/

There was also a leaflet enclosed called "Medication Changes: A Guide for Patients". I've just tried to find it online. The one I have is from HealthEast which seems to specify to my area so I suspect that if they're going to make changes, your "healtheast" equivalent will speak up for their own area (may explain why it does not seem to be online)... maybe.

If we want me to type it's details in, just say and I'll do it next post. Smile
The reasons are along the lines of:
~new safety concerns
~new evidence about the benefits of a drug
~new treatment guidelines

I assume the changes I've had come under the new treatment guidelines...

I normally have a bad case of dawn phen, but since on a diet and having little or no carbs for dinner this seems to have gone away, don't know if this might help others struggling with high morning BG.

Normal diet might be:

Breafast: Doorstep Toast and marmalade
Lunch: Baguette with proper butter and pate or Bacon sandwhich
Dinner: BBQ, loads meat and fish (bread rolls for burgers or potatoes) with salad

Which do people prefer the sound of?

They both sound good!

No problem at all - why would it be? Smile

In the spirit of sharing information...

Breakfast - Bran flakes, blueberries on top and a protein shake on the side
Lunch - M&S New York Pastrami Sandwich (en route to a wedding)
Dinner - Smoked Salmon, then Roast Lamb, then mini cheesecake desserts (at wedding)

Not a typical dinner for me though - was a treat!

Thank you Simon & good to see you've joined in.

Love the sound of your dinner at your wedding especially the smoked salmon. Hope you had a great time at the wedding.

Topic: Carbohydrate Counting / Thai food

It will be hard, unless it is being made yourself................but its so tasty..........

Prawn Toast would follow the normal bread carb values, although the fat content would delay absorption....
Spring roll would probably be about 1-2 CPs....
Dim Sum do have carbs, they would be hard to calculate, but just over do it......
Rice and noodles have carbs too, I usually just compare the amounts to a take away container, which is about 7-8CPs.......

It always a guessing game, and you wont get it right all the time....

Seen this somewhere else on the internet and thought it would be good to start a topic on here too.

It can be helpful sometimes to know what sort of things people are eating, as it might give others some ideas for a more varied diet. So, here's mine for yesterday:

Breakfast - cheese on toast (2 x rnds)

Lunch - tuna & mayo sandwich (2 x rnds)
cup soup

Evening meal - Poole's meat & potato pie

Embarassed not the healthiest things i know!

I find it impossible to try and judge the CPs in this. For starter I had prawn toast (small triangle), mini chicken leg, mini spring roll, and mini dim sum. And I mean mini. Then we had 3 courses between both of us and a portion of rice and noodles between us. The rice and noodles does not amount to an average portion you get say in a tray from a Chinese/Indian.. I totally miscalculated and ended up at 16. I presume it is the sweet sauces that this food is in that does it because most of it is meat and veg.

Topic: General Discussion / NHS Medication Changes: Are you affected?

Thanks for your replies. Smile

I think you have a very good point marke. I only just started using the machine this week and it's been so far so good (with prescription and machine use). I'm so glad it came with a testing solution - I had a hypo the other night where I did not seem to have any symptoms at all. Sad I must of tested it about 5 times to make sure while putting wine gums in my mouth... >_>;;; It seemed to be telling the truth...

marke said:
Yes the test strips do seem to be a lot cheaper, but why ? Being naturally suspicious I would question the motivation both of you doctors and the company that makes the meters/strips in singapore. What happened to freedom of choice in the NHS ?

Sadly, what my DAFNE nurse said is how it is - "Some surgeries are changing patients to these meters as the strips are cheaper than most others on the market but are just as reliable/easy to use". One of the diabetic nurses at my GP also confirmed this. It does seem to be something they're just moving people on to. Personally, I guess I'm okay as long as I can still get stocked. So far I have done so I'm not overly alarmed yet. I just need to use it for testing.

On that note, even my pharmacy (they're in the same building as the GP surgery) commented about my prescription being another one asking for iCare strips... and that it seems to be a good machine... obviously they weren't made aware either but they had the strips in stock. :/

marke said:
So will they put in place the infrastructure to support this equipment, rather than the current very basic website with no support and only one phone number for the UK. What happens if you lose or have problems with the meter ?

I'm not sure what to say on this one. I've never ever had to call for a replacement machine - not that I think it won't ever happen. If it did, I'd be more inclined to speak to my diabetes teams than the company itself in case they had one spare handy.

marke said:
Do all the chemists in your area stock these test strips and can they get supplies easily. You would hope that the PCT/consortium would have put all that in place before forcing you to use the meter, but have they ?

How do we find out about the PCT/consortium? Is that public information? I hope so - makes sense to be considering it's primarily a public company...

Edit: I signed up to their forum today - http://icareadvanced.com/forum. It seems to be quite a broken website. Sad A thread I posted did not appear and I was told that I did not have permission to access "this page". I have PM'd the admin... let's see if they do anything...

Edit: It looks like it's fixed! Smile

Lantus doesn't take 24 hours to 'kick-in': http://www.dafneonline.co.uk/hbook_topics/202

It should start working within a couple of hours according to the DAFNE handbook (I don't use Lantus) and has a length of action of up to 24 hours...so keep an eye on your BG levels during the day today and you should be relax a bit more later on.

Jayno said:
So far so good! Woke up with BG of 3.6 but doesn't lantus take 24 hrs to kick in so I'm guessing from 6pm onwards today I'll get the problems?
I have the timeinsliun pen top which I was using and then I got moved to lantus and they don't do one of that make!

Topic: Questions for HCPs / time change of BI

Hi Anna,

The advice in DAFNE is to take your BI at bedtime and as you get up in the morning; this is particularly important with an 'isophane' insulin such as Humulin I which as you rightly say peaks around 4-6hrs after injection, but then tails off.

The added problem in the morning is the Dawn Phenomenon, where your body releases strong hormones as you wake, resulting in a rising BG. If you have taken your Humulin I too early (e.g. at evening meal-time), its action has pretty much worn off by then and doesn't counteract the hormones, so the later you can take it at night the better.

You might find you need an increased dose though, but don't forget to do 3am BG tests before and after you make increases to your bedtime BI to make sure you have the leeway to increase it (i.e. that it's not causing your BG to drop below 4.5mmol/l at its peak).

Good luck,

Carolin

Hi,

Which pen/lantus are you on? I'm on Levemir so no idea about it, but just looked on the Timesulin site and they do a lid for the Lantus Solostar pen (http://timesulin.com/how-it-works/pens).
