Topic: General Discussion / Apple software updates

Hi,
Sorry what I mean was type something like 'libre 3 ios 18.7.1' into a search engine ( can be good or any other search engine) and read through the results. Like all search results some are positive , some are negative and some don't apply. It just gives you a feel as to if you are likely to experience issues. Obviously you need to ensure you enter the correct Libre you have in the search, also make sure you don't have a libre App update waiting to be installed.

I tend to use https://www.reddit.com/ for a lot of my info. They have forums for pretty much everything and have various Libre forums which might give you answers. The general opinion on the Dexcom forums is iOS updates for minor versions i.e 18.7 to 18.7.1 are always ok to update, its iOS 18 to ios 26 that are much more risky. That said various people on google say they are running iOS 26.0.1 with the Libre 3 app ok. I haven't updated my phonme to iOS 26 yet though Laughing

It's not just you ! the Reddit forums are full of people moaning about Abbott and Dexcom not updating their software inline with operating system updates. The best option is to keep googling for your Libre version and the iOS version and see if other people have updated to the iOS with no problems. This is what I do, I always wait a few weeks after I find people posting one way or the other to see if many have problems. If everyone seems to be fairly ok after updating I update.
I at least have a fallback now as my pump also links to my CGM and pumps are updated once every few years if you are lucky so if I did update my phone and the app stopped working, I still have the pump giving me readings and warnings....

I am beginning to think it's just me, however, Abbot have at long last just announced an update to 18.5 but I can only download 18.7.1 or 26! So it appears that once Abbot have got around to updating apple have moved on so you can't go back and sync with the latest Abbot update!
Sorry if I am making a meal out of this but surely this must affect many others?

Topic: DAFNE Online Mobile / iPhone software updates

Hi,
Yes there are lots of people in the same position and its not going to get better any time soon :-( The issue is all CGM apps have to be verified as medical devices by Medical Regulatory authorities. Back in the day medical devices were self contained, I had an original Freestyle Libre and that came with a 'reader' device. Dexcom still sell a 'reader' as well as providing the app. The readers never get updated and so are only verified once. Phones on the other hand are constantly updated and although the apps are not changed the authorities insist that because the underlying OS has changed the whole verification process has to be done again. Whilst you can sort of see their logic, its based on a different world and doesn't reflect current reality, however its very unlikely that they will change their approach anytime in the near future.

I have been waiting to update my iPhone software but Abbot have been on 18.3.2 for months for Libre 2 Plus. There must be many others in this position?

Topic: General Discussion / Question about Background Insulin

I'd suggest still taking a little bit of BI at night. The reason is that while Levemir can last up to 24 hours, it doesn't necessarily do that, and if you are only taking it once per day, then there may be a period where you have no insulin on board at all which can lead to DKA.

Even if it is only 1 unit at night, then at least you will still have some present in your body. You can reduce the amount injected in the morning accordingly, so if you still plan on taking 10 units per day, then try 9 in the morning and 1 at night. If you still get overnight hypos, then try 8 in the morning and 1 at night.

Hope it works out for you

Topic: General Discussion / Question about Background Insulin

Hi,
Thanks for your reply.
I am sick of the low sugars at 2am. I don't know how much BI too take. I only am taking 10 total units, 5 in the morning and 5 at night. still go low, and I know 5 in the morning is not enough. Today I had a funeral at 11am, so I took a 2cp snack to ensure I never went low , and at 2:30pm my sugar was at 16.7.
Would I be best taking only one injection in the morning, instead of splitting the dose?
Tonight I will take 3 units of BI and tomorrow inject 10 units in the morning and none at night

Topic: General Discussion / Question about Background Insulin

Looking at this, I'm thinking that Wednesday through Sunday were actually very well-controlled. Apart from the hypo on Saturday morning, you were largely within the range that you want to be in. Well done.

What readings in particular are worrying you? I'd actually be very happy if my control was as good as this.

Hi Warwick,

Thank you again. As you can see my BS is all over the place. If I lower BI in the morning, my sugars are high, have 3cp snack at bedtime with no insuln, I go low.

Topic: General Discussion / Question about Background Insulin

You seem to be experiencing two issues here - lows during the day and overnight lows and I think they have different causes.

With regards to the overnight lows, they all seem to be within the 4-hour action period of your QA. You appear to be super-sensitive to insulin (Sat 05.30 = 18.9, Sat 07:45 = 4.4 after only 3 units of QA is quite extraordinary).

For the night time, I think that the amount of QA that you are having for the CPs probably needs reducing. So for example, Friday 23:00 4 CPs with 4 QA, perhaps try 3 QA or even 2 QA for this in future.

Daytime hypos, especially the Friday and Saturday ones around lunchtime aren't within a 4 hour period of QA, so that would seem to indicate that you BI might be too high in the mornings. You could try reducing that from 4 units to 3, and see after a few days whether that is stopping the low BGs around lunchtime.

DAFNE relies on getting BI correct, and then making any necessary changes to QA after that, but to work out whether we are getting the BI right, we really need to have periods of around 8 hours without any eating or QA injections.

Obviously, treat any hypos that occur, but if it is possible to have an 8 hour period of fasting after an evening meal, then you will be able to see if the BI is sufficient or too much.

To summarise:

1) If eating before going to bed, reduce the amount of QA that you have with the CPs. So if having 4 CPs, try 2 or 3 QA rather than 4.
2) Try reducing your morning BI from 4 to 3, and see if that stops the lows around lunchtime.
3) Try and have an 8 hour period without food overnight. Four hours after your last meal, test BGs, and then 4 hours later, test again. If you BI is correct, then the BGs for these two readings should be within about 1.5 mmol/L of each other. You can also run this test during the day if you want to check whether your daytime BI is correct. Have breakfast and dinner, but skip any snacks and lunch.

Hope that is helpful

High doses of Levemir last for around 24 hours. Lower doses last up to about 19 hours. This is the reason that it is advisable to dose twice daily just to ensure that there is always some present in your body.

Levemir has a pretty flat profile, so you could probably second-dose at any time between 3 pm and 7 pm that is convenient for you, but probably best to try and inject around the same time each day to avoid hypos (up to a 90-minute variation probably won't matter too much).

Topic: General Discussion / Question about Background Insulin

Hi,
looking for some help regarding BI, please.
I inject Levermir twice daily, first injection when I wake in the morning at 5am. My question is, if i wake up at 5am, when should I
inject my night BI time dose?

If you are travelling, then I recommend using a Frio wallet - https://friouk.com

I bought one when I was first diagnosed 15 years ago and it is still working well, and keeping my insulin cool in hot climates. Even on a 43 degree Celsius day in Melbourne, my insulin stayed cool and effective

I keep mine in the top part of the fridge and our fridge like all fridges varies its temperature depending on how much its opened Smile . It doesn't have to be precisely 2-8 degrees, you just don't want it to freeze, hence it should always be in hand luggage when flying. Keeping it 'chilled' just ensures it stays effective until taken out of the fridge at which point it should last for around 28 days. However like blood tests this is just an average, it cluld last longer it could last shorter. Nothing in Diabetes is precise. Very Happy

Topic: Site Development / Course Handbook

Hi,
Sorry no we are not allowed to provide copies of the handbook as part of our agreement with the DAFNE Programme Team, we can only provide access to a copy that cannot be downloaded or printed.

Can you check in firefox under the site data again, when you filter to dafneonline.co.uk , you should have 4 cookies and mine has 4.6Kb of storage. When I clear the data, I always save and then restart the browser to ensure it has nothing still hanging around. I then open the handbook and it works everytime.

Regards

MarkE

Topic: General Discussion / Insulin storage

I understand that unopened insulin should be stored at a temperature of between 2-8 degrees, however, I find that fridge temperatures tend to vary, especially depending upon where you place the insulin in the fridge

I just wondered how resilient insulin was to changes in temperatures especially when travelling abroad and what other people's experiences were with storing insulin

Thank you in anticipation
