Topic: Questions ? / Fitting when hypo

Puts in perspective some of the moans we have about being Diabetics. I would imagine having fits all the time is really not a pleasant experience. I would have thought in an ideal world they would have given you a Continuous Blood Glucose Monitor so you could maybe work out the 'tipping' point that causes you to fit. Generally most of us do not fit when hypo but some I guess are particularly unlucky and their bodies are more sensitive to low blood sugar and the effects it has. I guess you also get the double-whammy of a fit causing you to expend a lot of energy meaning you need more carbs to get your BG back up and to keep it up.
Thanks for sharing your experiences anyway, if nothing else it shows people that they are not alone in suffering a problem like fits and hypos

at the moment it is unlikely there will be an app that runs on a Nokia. You can however access the mobile version of the site on your phone if it has internet access. This is a cut down version of the site designed to run on mobile phones. Obviously the downside is you need internet access to use it. If there is anyone out there that wants to develop a Java based app that will run on phones like this, we would of course be interested in hearing from them Laughing

the lows are definately caused by the exercise. generally the effects of exercise will be felt for 24 hours afterwards. Maybe more, maybe less as we are all different. It is basically the fact that your muscles etc are still using more energy ( from blood sugar) than normal. I always had to lower my QA the morning after an evening at the gym, and sometimes my BI depended on the level of exercise. Sadly I saved money by not going to the gym and now just cycle as its generally cheaper ;-)
I think its also true that most exercise results in adrenaline being released which is a hormone that primes the body for action. Any 'action' generally requires more 'energy' than resting so more sugar in the blood to provide the energy. Sadly being diabetic means you don't get the corresponding insulin boost to process this blood sugar. However injecting extra insulin is also potentially dangerous, so probably best to do exercise when there is some QA in the blood but not injected specially rather as a tail off from a previous meal. Of course the difficulty is the only way to work out the best solution for each individual is by trial and error. Still nobody said being Diabetic would be easy even with DAFNE Very Happy
I would also be careful about moving your Levemir to that much earlier. Generally having Levemir before bed and when you get up is it gives you a greater overlap in the morning when generally your insulin requirements tend to be higher. This is due to dawn phenomenon type effects if not the full blown thing. Instead you would be more likely to get an overlap in the evening. This might have the effect you require, but could also leave you higher in the morning. All could's of course because we are react differently.

Sorry it seems that in Scotland you don't have a PALS service Sad There is a group in Scotland called Local Health Council which has a wonderful website full of marketing and hollow promises but no real substance. I also tried googling for podiatry and diabetes in Glasgow and got very little, more than I little worrying. Do you not have Podiatrists who are also Diabetes experts in Scotland ?? There is supposed to be a national Diabetes Framework and number of 'standards' that PCT's are suposed to meet and footcare should be amongst these. I would agree with Novarapidboi surely your Diabetes team can give you a number for the podiatrist, how can they not have one when its a basic part of diabetes care. If any of our other graduates from Glasgow have any ideas please let us know, this is an unacceptable situation.

You should check with your diabetic nurse or local PALS team ( should be a link on your PCT's website) how you get an appointment with your local Diabetes Chiropody team. They should be more than happy to see you and check your feet. It most likely has nothing to do with your Diabetes however because we are at 'increased' risk of foot problems it is better to get it checked out by an expert. My local chiropody team has always been keen to emphasise thst you should see them if you are in any doubt, better to waste their time with an unnecessary appointment than ignore a problem that could become more serious if left untreated. It is part of the 'standard' treatment you should receive as a Diabetic from yuor local Diabetes Team, so make sure you do !

Topic: DAFNE Online Mobile / Download entries from site?

hi, any idea how many entries you are trying to download and are you on WiFi or just the phones 3G signal ? Simon is the expert on this but he is currently on holiday until the middle of the week. I have just added a single entry on the site and downloaded it to my phone and it did it pretty quickly but I am on WiFi.
I suspect it is more likely the number of entries it is trying to sync but as I say Simon is the expert and we will probably have to wait for him to look into it.

But seriously rebound hyperglycemia is the idea that if you have a hypo in the night that in the morning you wake up with high blood sugars because of the Hypo. This is now considered to be unlikely and the idea is to look for other reasons. Thats a quick take on it, I'm sure when they send us the info to put on the site it will be explained a lot better than my feeble attempt.

Due to the expansion of what was originally the Monklands DAFNE Centre we have had to rename it NHS Lanarkshire since it now consists of 3 hospitals not just Monklands. Apparently it was confusing new graduates have the centre called Monklands. So appologies and I hope it doesn't confuse people !

Hi, yes the version on the site is now the latest one being issued to DAFNE graduates which is called 'version 9'. In this version the apple weight value was changed from 120g to 100g. Unfortunately I cannot tell you why this is, my part was to convert it to a form to be used on the site. I wasn't told why certain foods had their values changed. I would presume that generally 100g to 1CP is more consistent.
Remember its just a guide rather than a hard and fast rule. I will ask if there is anything that explains why values where changed and post it here if I am provided any information.

Hi cathy and welcome. If nothing else you are testing regularly and that's a big bonus. We are all bad from time to time no one is perfect but at least now you have the tools to improve things. As for the other nurses about time they listened and stopped preaching. In every job we all have new things to learn and never know it all even if we think we do.

Simon made a very good point that I had forgotten about, which is surprising considering the circumstances. You need to be careful when you stop taking the steroids. They will taper them off slowly so that you body resumes production of 'natural' steroids. However my insulin requirements became a bit unpredictable for a week or two after I had finished the steroids completely. I had my only ever assisted hypo during this period. I was at work and apparently started behaving strangely then went to sleep at my desk. Sadly despite all my previous instructions to colleagues they called an ambulance. The operator told them to give me coca cola. I remember waking up and thinking oh there is an ambulance crew here, I wonder why. Oh sh*t it's for me !! I have been fine ever since.

I have experience but no simple solution. I have been on steriods a number of times and experienced insulin resistance as a result. The only solution I found was trial and error, however I also found that the larger the meal I ate, the harder it was to adjust for the resistance. It didn't seem to be linear i.e if I at 4 CP's and used a ratio of 2:1, if I ate 8 CP's then a 2:1 ratio was not enough. The only solution I found was to try and keep the CP's down and maybe eat more often and inject for each. Another important factor is to increase your BI accordingly as well. This will require acting like you are starting from scratch and try to get your BI right with carb free days. It won't 'fix' the issue but it will improve it by giving a better baseline to adjust the QA for.

Your initial resource for obtaining Blood Glucose Diaries should be your local DAFNE Centre. Your local DAFNE Educators should receive supplies of DAFNE Blood Glucose Diaries.
If your DAFNE Centre does not have any copies available or you have lost contact with your DAFNE Educators, you can obtain copies for a small fee from the DAFNE Programme Team. The best way of ordering them is by using the DAFNE Website.
If you go to www.dafne.uk.com and then select ‘People with Type 1 Diabetes’ from the left hand menu, followed by ‘DAFNE Shop’. Here you can order a number of DAFNE related items including Blood Glucose Diaries.

Topic: General Discussion / SE KENT PCT DAFNE scrapped

All, If anyone reads this, we need your help. The wonderful South East Kent PCT has decided to stop DAFNE in favour of BERTIE. The arguement is it is only a 4 day course and so they will be able to offer more courses. This seems more than a little unlikely. As ever I suspect its a 'done deal' but if like me you are unhappy about this change and are a graduate from South East Kent, please respond to this thread. I intend to contact the local PCT and the local MP to ask some pertinent questions, such as what ongoing support will there be for DAFNE graduates, how do they explain that BERTIE is not fully QUIPP acredited despite the PCT claiming it is.

I would be happier if I believed that this was being done for the benefit of Diabetics within the PCT's area, but I suspect it is just a tick the box/cost cutting exercise.

I hate to suggest it, but a 3am test might be useful. It might help pin-point what is happening. You 'could' be hypo-ing but I would suspect that is unlikely. It could be 'dawn phenomenon' but it has been suggested to me that usually you have this from day one rather than it just starting to be an issue. However we are all different and there is no 100% guaranteed solution that works for everyone. It seems unlikely that your BI is not enough since you are on a single dose and if it was going to run out it would be in the evening and not the morning, also Lantus is supposed to be a 24hr insulin. It also seems odd that just 3 units brings you back in range. For most of us if we are that far over it would take a lot more than 3 units to get us back in range. You really do need to talk to your Diabetes team I suspect to help work out what is going on.

Hi, there is a link at the bottom of every page that allows switching between the full and mobile sites. With regards to text entry on a mobile, unfortunately this will always be problematic due to the way mobile phone web browsers and text entry with them works. This is not an issue with the site rather the way the phone works and so is outside of our control. You can try it in both 'views' of the site to see which one is better. We can try experimenting with the layout of the mobile site to improve the 'jumping' but entering text will always be down to the phones implementation.

Hi Simon, I'm a bit confused by your response. Your GP does DAFNE ? Generally DAFNE is run at DAFNE Centres which are hospitals like the one you did your course at. Hence my confusion by your reference to your GP doing it. Do you mean they do something similiar to DAFNE or that they actually run courses under the umbrella of one of the DAFNE Centres ? Where is the GP in the UK ? Just curious Smile

hey, I'm not the only one !! I blame the xmas pudding, I know I should resist it but just cannot. Despite injecting more than I thought I needed I still ended up at 18.7 in the evening. Its good to know I'm not the only one who still gets it wrong at times like these.

If you are a Diabetic that is interested DAFNE and live in or close to one of the trial areas (London, Cambridge, Sheffield, Harrogate, Dumfries & Galloway, Glasgow, Edinburgh), this is the place to go :

The page above is about the DAFNE REPOSE trial. This is basically a trial is to find out if pump therapy is any better than multiple injections when people have intensive structured education through DAFNE courses. . It is hoped that the outcome will identify the added advantages of insulin pumps, the people who are most likely to benefit from them and ultimately inform NICE criteria for pump therapy.

Topic: Site Development / Browser Remembering Credentuals...

Hi, This is a 'client' issue not a server issue. By this I mean password etc are stored by the local browser NOT the site, so the problem will be somethnig to do with the browser. I have not really used firefox portable so its hard to comment, however I would go into the password manager in the portable version and make sure there is no entry for dafneonline. If there is I would delete it and then try to get firefox to remember the site again. Googling it there does seem to be a number of issues with firefox portable and passwords.
Personally I have to say I do not use the save password feature and would never use it on the portable version. This is probably because I am paranoid about security and would
not trust it. If you lost the USB stick with firefox portable on, it would only take someone with security knowledge minutes to break the password on your password file and have access to all your passwords and the sites you use them on. But its your choice what you do with your data Very Happy

sorry to disappoint you both, but the DAFNE App is written specifically for the iphone and Android platforms, its not java based. The issue is the 'cost' and effort of developing for what are unfortunately 'niche' platforms. A blackberry is more of a business phone and I'm presuming ( I will check) that to develop for it will require costs and expertise that
we don't posess. We have the other versions of the app because we had both the expertise and software to do it. If there is someone signed up who has the required skills
and software to develop a blackberry app then we are happy to share the information and help develop the app. Our only requirement is that it is free. This site is 100% not for
profit, we are not here to make money, rather give back some of the benefit we got from DAFNE.
