Topic: Questions for HCPs / Type 2 & DAFNE

My husband was diagnosed as a type 2 at the age of 40. I did the Dafne course as I am a type 1 (and by the way I take metformin as well as insulin) and he did it by proxy along with me. He has now actually been tested with the GAD test which has actually proved he is in fact a type 1.

guys, Just a word of caution. I would advise against putting your email addresses in messages on the forums. Not because of anyone on here, but because spammers scan websites for email addresses to use to send spam to. Since these are public forums, anyone or anything can access them and get hold of your email address. As someone who works in the IT industry and has to battle to block my users receiving spam I can assure you its VERY annoying to start receiving large amounts of spam AND also to have your email address used as the apparent source of spam.
You can use the site private message facility to send each other 'private' messages that contain your email addresses. That way they are NOT available publicly to be accessed by spammers. To use private messages just click on 'message inbox' in the right hand menu.

im sure all waiting lists aintthe same Sad i went to my first diabetes education centre for the first time eva and was on DAFNE in about 5 weeks and also with having type 1 for 15 years but it helped me loads for thinking i knew it all (lol) great cause and hope your friend has better control ASAP

Hi Guys,

I have quite a marked dawn phenomenon and was almost at the end of my tether until recently. I like yourselves started of checking for night-time hypos (as to be honest this is what I thought it might be) when I discovered that from 4am-8am I'd creep up from 7 to 17!! Despite many attempts to correct this I just couldn't get it sorted and when I increased my BI to a certain amount I went lo which ment I ended up being high in the morning anyway. Nightmare! It was suggested that I get an insulin pump but to be honest I hate the idea of being attached to a machine so I thought I was going to have to just out up with it and correct every morning.

However, recently I was put on Insulin I for my bedtime BI (the same as the old insulatard) which action peaks at 4 hours meaning if I time it right (ie take my bedtime jag at midnight or later) it combats the increase in BG and as a result my morning BMs have been really good. Not perfect but it is a hell of a lot better than it was (down to single figures mostly). The only draw back is you really can't take it before midnight otherwise it will peak to early and you will go lo (but if you are an owl this is perfect, or just set your alarm if you want an early bed).

This also means that you would have to split your background dose too so you'd have to take another jag in the morning of one of the new, slower release ones (eg levimir, lantus). I have to say though for me it has made a huge difference and I don't feel so disheartned with my control.

Its definately something to think about!

PS - If you have a dawn phenomenon you are more likely to need a higher ratio at breakfast anyway to combat that no matter how fit you are as those hormones that are released to wake you up (which gives you the dawn phenomenon) are still hanging about your system. I've found that even if I wake up fine and don't eat breakfast I still have to give myself a couple of units to keep me level for lunch.

Hi John

A heinz tin of lentil soup has a few very small squares of potato I don't even know if there is 10g worth to count but I count 1cp plus the tomato puree a bit like following the rules of beans. I'm looking at a tin just now per 100g is 7.7g a full tin 400g so it works out as 3cp. I've found it doesn't put my BG up much and in the past I've taken my full amount of insulin ratio 2:1 so 6units. I've hypod 1hr later. So I just half it. Taking 1.5 units to cover the tin of soup.

When I said never hypod I mean I've counted the right amount of carbs/ taken the right amount of insulin. I've never hypod when cooking homemade food because I've counted my carbs correctly and taken the right amount of insulin to match the carbs, so I'm counting right! I've not over estimated in my cps basically therefore no risk of hypo.

Since following DAFNE rules my hypos have reduced dramatically and that has a lot to do with weighing my food and counting carbs properly, the system really does work. It's all second nature now!

Hello Sam ..

From what you have said, you already work in the way that will suit the form fine ..it will just provide a standard layout for your calculations to keep. Like you mention, In my own use of the form I also tend to favour portion size at he unit rather than gr as its easier to relates to eg half of a Morrisons Steak Pie is 6.5 CP, half can rice pudding is 3.3 CP as well as dividing up Pat’s home cooked recipes into half or third portions.

Just a detail query from your potato comment .. why half the CP for the potatoes in the soup .. just from experience?

And if by ‘never hypoed’ you mean you have never had BG less than 3.5 then you could teach us all a lot, that’s for sure. Suspect you mean never been down in the 1.somethings?

I'm curious ..your form is there anyway I could get a copy? I in the past had difficulty in over estimating my CPs as every food tends to have carb nutrition info on the packaging. When attending the DAFNE course we were told to watch what we count for example a tin of lentil soup although a heinz tin from memory works out at about 3 cps I normally 1/2 it for all the potato that's in it plus lentils are in low GI. You end up taking too much insulin to cover and could hypo later on. I guess it's a case of what ingredients are in the foods etc and would you count it. My portions of home cooked meals can depend on how big or small my portion is. I make a chicken pie which has puff pastry which is high in carbs but I love it sooo much I get 4 portions out the whole pie. You could get so much more I guess what I'm trying to say is say one of my recipes is a total of 400g carbs i would say to folk divide by the number of portions you want out of it, everyones appetite is different. I would also list all ingredients and mark the carb amount per that item and it would be up to that person whether to count it or not. I kile to work out the carb amount for each food item, total it and then divide by the number of portions I get out of it. I take my insulin accordingly. I've never hypod after which is a good sign. I guess it's just trial and error.

Hi - just one note of caution.
Please don't be too heavy handed with your friend. We all need to be in control of our own destiny and things like diabetes can leave you feeling like you no longer have control over that. Other people doing things behind her back is not necessarily going to help her feel in control again.

All your friend needs to know is what resources are available to assist her and that people are routing for her and then she will, in her own time, make the choices that work for her and allow her to regain control of her situation in life. If you are frustrated with the time it takes her to come to terms with her condition, imagine how much more frustrating it is to realise that you're in a situation that will never end and that you are going to have to find the best way you can to manage it for the rest of your life.

Take a look around this site and you will see that people who have long ago (decades) come to terms with their condition are still working out what is the best way to manage it now. It can be a constantly changing situation, so please be patient with your friend and allow her to reach her long term conclusions and choices in her own time frame.

I've just e-mailed my DSN after doing 2 days of no carb breakfasts and being very good about not changing anything except my a.m. BI. I will definitely let you know how I get on - only hope it's weeks rather than months. :-)

Well I have my next appointment on 20th November (eek) and know I'll get a row too - but really hoping that filling it all in online will really inspire me to pay more attention to my diabetes!

I got invited to my cousin's wedding in December and there was a bit on the reply card to state if you have any special dietary requirements - I didn't fill it in. I then had my auntie on the phone asking if I'd forgotten to fill it in etc..! I wish people would realise that in fact we can eat what we like!!

I know what you mean about drawing attention to your diabetes - I hate testing or injecting in front of people I don't know. So sometimes it doesn't get done to be honest!

Hi Lynne,

I did the course approx 6-8 months ago after struggling with poor control and poor health for a number of years. My weight spirallled out of control and I crept up to 18st 9lbs. My health was starting to affect my life style.

DAFNE taught me a lot; especially about food generally. It really is worth the effort. As for me??? Welll

My weight has dropped to 14 stone, my GP has taken me off ALL my blood pressure tablets (Lisinopril and Slozem), I have had zero Hypos, I am generally quite fit & healthy , I take much less insulin now (sometimes even skipping my long acting dose depending upon my carb intake) and I eat like a horse.

As for keeping records etc I suppose everyone finds their own way. I kept good records and counted accurately for a while; now it is just second nature, I sort of keep it in my head now....I find I do not need to keep accurate records..I just sort of got "into th groove" if that makes sense.

It is well worth perservering ...a couple of good books/reading some on line forums helps.

Hi Lynne, i did DAFNE training at St John's in March, like you i think it's brilliant, but filling in my diary is just a pain in the ***!
I got into trouble at my clinic appt in sept for not filling it in, my hb1 count had went up as well, so as my 'punishment' i had to fill my diary in for a few weeks and send it to Liz, i haven't filled it in for a couple of weeks, and find it so hard to start again.
I find that it's easy for people to tell you to do it, but they are not the ones doing it, i don't want everyone knowing i am diabetic as it is, and this just draws attention to the fact. I get a half hour break, so i have to test my BG, work out my CP's eat and fill in my diary, i am also coeliac so have to make sure i don't eat anything with gluten in it. I feel food is becoming a BIG issue with me!
Do you find that non diabetics 'tell' you what you can't eat? i get it all the time so that's why i hate telling people i am diabetic.
I know it's helping me but it's really hard, so you are not alone!

I did the DAFNE training in May and I thought it was brilliant - really easy and more importantly it actually works!

But I have a severe case of the 'can't be bothereds'!! I know what to do, I'm just not doing it.

I need a major kick! Is there hope for me??

Topic: Questions for HCPs / Rising morning BG knowing the 'Dawn' Phenomenon is not present

Hi Everyone

I have recently been doing throught the night and early morning BG tests and have discovered that a rising morning blood sugar after morning Lantus at 7.30 am and before breakfast at 10.00am ish not due to our friend 'Dawn'. Can anyone explain what else it might be, if all other areas seem ok, ie. ratios, morning Lantus checked with CHO free lunch etc.

Sounds a great idea to encourage the trying out of new meals, with the added attraction that Meal CP is already calculated for a sized portion.

As far as the CP calculation itself for a ‘home cooked’ meal, the Meal CP Estimator form (which some of you already have had from me) seems to work well when my partner Pat makes a ‘from basics’ meal with lots of ingredients.

The form helps ensure all ingredients are correctly taken account of and prevents CP calculations from being pencilled on Pat’s recipe books… not a good idea I am informed!

If the recipes were perhaps structured in a similar manner, showing ingredient CPs, cooks using could adjust the mixture to their own preference and easily see the impact on the overall CP.

Could well, however, be best to start simple, as Sam suggests, and see how much enthusiasm there is out there and enhance if required.

Bearing in mind the old 80/20% rule, to make it fly perhaps the submission of a new recipe should be a condition of membership of Dafne Online. But Beans on Toast should probably not count even for us mere males.

You should let her diabetic support team now she could attend at short notice if this is the case.
I did just that and got a call the following week as somebody had to drop out.
