Topic: General Discussion / Four O'clock Follies (or why does my BG fall off a cliff at 4pm)

Dear Assembled Pancreatic-ally Challenged,

As DAFNE-ilise by CP ratios and tweak my BI levels, as I've got my BG levels back to "my feet won't fall off" levels I've noticed a strange problem, my BG levels really start to drop at around 4pm. I can see from my Libre that everything is stable up to them, then it falls off a cliff (a 2m/mol drop in an hour).

Any one got any bright ideas as to why it might be happening? And solutions (ideally ones that don't involve a new regular 4pm Krispy Kreme)

Never managed to find a decent solution to the stress problem, besides asking work for reasonable adjustments as part of my rights under the disability act. The best thing I ever did to be frank - quality of life greatly improved. Work is never worth your health and wellbeing so you're well within your rights to ask for help if you need it.

Hi Sue,
I surely hope your dire situation is resolved satisfactorily. If they are having a recall of strips for the Aviva that you rely on for medicinal quantifications to inject insulin the company must have an ethical stance to supply you with replacement strips to use. In Australia the glucometers are only given out by hospitals with specialists recommendations so the strips and machines even take on more significance for your health. In the past, I have found the service desk at Accucheck helpful and willing to exchange my Aviva strips and glucometers after discussion. However it has not been a mass recall. Give them an email to get replacement strips when returning your other suspect ones.
Furthering the discussion on Aviva strips I have always found up to 3 to 5 strips in every packet stuffs up. I used to think due to the government subs on the strips that it didn’t matter much yet it is 10% of every 50 strip packet that is useless. Not good enough so I have been ring up the company and getting replacements. I wouldn’t want to be on my last strip in the community that doesn’t work! Is it the usual practice with large multinationals that we never really seem to get the full picture? Have any other DAFNE users had the same problems with their strips?

Has anyone else been left without any test strips as a result of the recent ACCU-CHEK Aviva test strips product recall?
I was notified at 5.58pm on Friday about the recall. All my test strips were from an offending Lot. I was left in a very difficult situation.

They did mention the subject very briefly in our Libre familiarisation talk but I think I'll take your advice as I was all over the place a couple of days ago when I tried 50/50! :-(

you will get valuable information from the trial and error approach...

see what your diabetic nurse/doctor has to say maybe...

Thanks Helen, Fruit is certainly a guessing game at times but I will try the carb count next time I eat this delicious fruit!

Thanks for your advice! I have ordered a Silhouette infusion set and look forward to using it! No more "insulin blocked" alerts with the latest replacement pump! Yee haa!

Hi Jo, I have had a 640g for a couple of years and have had several "insulin block" alerts. Mine appear to be bubbles in the reservoir! I run about 10units through pump (plays havoc with the data) and then reposition the pump ensuring the pump is upside down (I wear it in a pocket not using the clip...this puts any bubbles at the bottom away from where the insulin comes out). Very occasionally it is the infusion set...a kink in the cannula. I also use the Silhouette and have had very few issues. Have not ever needed a loaner pump from Medtronic. Helen

I have had Diabetes for 64 years and have been a DAFNE Graduate for several years. I did not use this site.
I have just been on a Refresher Course which I found most helpful even after 14 years.
I am interested in getting involved . I am a pensioner and have other disabilities and am limited as to
what I can do.

Hi thanks for your early reply!

Yes I was DAFNE trained three or four years ago, but never paid much attention to the problem before I went on to the Libre system as it wasn't very obvious then what was happening.

I am thinking of things like pizzas, fish and chips and especially things like German Bratwurst sausages. I seem to react a lot slower than you do though as I would have to wait at least a couple of hours before taking the second part of the bolus - bed time even!

That's why I'd like to have a look at some official advice before I start experimenting on my own! :-)

As with everything DAFNE, trial and error is the best approach......

Before the pump, when I had pizza, I started by splitting the dose 50/50 and then taking the second dose 30 minutes after eating....

this stopped the high later but it wasn't perfect....

the truth is with all the other hidden variables it doesn't always work......

so to come back to first point, trial and error is best.....

what specific foods are you thinking about....?

are you DAFNE trained?

Just wondering how DAFNE treats red papaya (paw paw) In some carb counting sites they say 140 grames = 9.7 grms Carb but in other places it says No carbs??

you could always try another brand if they are on offer..




I am going to try another infusion set (Silhouette) although Medtronic said there was nothing wrong with the infusion set I was using!

Hi Marke,

I have been in contact with Medtronic and after trying to problem solve they have sent me a replacement loaner pump twice as they said there was nothing wrong with the infusion set (Sure T) and this morning I connected the third replacement pump!! So far so good!!! Fingers crossed it will be third time lucky! I honestly would not have bothered with a loaner pump had I realised how much stress it would cause me! In January I will qualify for a brand NEW pump so I'm looking forward to that. Laughing

Hello, I started on the Freestyle Libre system about 6 weeks ago and have found it very revealing, causing me to revise my opinions on how to deal with many foods.

I have found that fatty foods cause a delay in the effectiveness of the fast acting bolus and would appreciate some guidance on how to split the bolus so I don't end up taking extra CHO at bedtime! Are there any links to available articles on this subject?

Sushi is quite easy to estimate actually, particularly maki. As you say there is some sugar in the rice but I find if I have six maki rolls it works out about 70 grams of CHO. Some of the starters are heavily sugared however and best avoided altogether - apart from the seaweed!

is it the same clip that goes on to the Veo?

I have had quite a few failures, although I have became a tad more careful with it now...
