I have diabetes retinopathy as well as a few other eye problems, all stemming from that though, I had the lucentus injection in one eye and unfortunitly ended up with one of the worst side effects and got a detached retina. I am completely blind in both eyes now. Not once did my diabetes clinic check my eyes in all the years I was with them, yet my sis is at same clinic and I remember her having hers tested. Hmmm. My sight went completely within a year of my own optician first picking up on the condition and he tested every 6months to a year.

Sorry to hear you've had problems with hypo's.

I wonder if that morning BI is actually too much, since you say you find your BG dropping low by late evening? As you know Levemir works at it's peak during the first 12 hours, but it's duration is longer, around 18 hours I think (from memory, don't quote me on it).

The only way to really know this is to test the BI, after lunch time looks to be the best time in your case, and means QA insulin won't be causing too much noise to see what the BI is doing. This may of course lead to higher BG initially and the need to change QA ratio as a result, but long term if it's going to be safer for you and make it easier to correct high BGs, I think it's worth digging into.

I think you're doing the right thing to play it safe with the overnight one for now. It can take a while to get it right as changes generally need to be made gradually.

Sorry to hear yluhave had so much trouble. I am very sympathetic to the problems you have experienced. I wonder what time you take your metformin. As you know, one's insulin resistance tends to rise for a while in the early morning, say 3 a.m. and metformin can help to improve one's fasting sugar if taken in the evening. I wonder if you would tolerate 2x 500mg in the evening. The advantages of adjusting this is that it does not tend to cause hypoglycaemia. It reduces the release of glucose from the liver at night. Of course, you would need to check with your medical advisers before making any such changes.
I note you are on a rather high dose of levothyroxine which acn affect one's blood sugar levels. The usual maximal dose is 200mcg daily. I know Epilim induces liver enzymes which may increase the rate of destruction of certain medications but I was not aware that levothyroxine requires a high dose on Epilim.
A few years ago I found it almost impossible to get basal levels right even adjusting by 1/2 unit levemir at night going from one extreme to another, but I've been much better on an insulin pump. Hopefully you will manage to get the adjustments right with patience.

Hi all,

I completed the DAFNE course in December, and, after a LONG time of having high BG's, was amazed to finally have BG's in the normal range. There were the few lows which i expected, and indeed which the HCP said would be a normal reaction to the changes in regime.

However, just after finishing the course, i had a severe hypo, which led to my partner administering the glucogon injection, and for the next few weeks, a super tight monitoring regime (usually 10+ tests per day)
Even after decreasing my BI at morning and night, and doing 1:1 for all meals of QA, i found i still dropped in the night-time, but then after treating, would wake up with super highs.
Wanting to sort this, i dropped my evening meal to 1/2:1 which resulted in a higher BG at night, (usually 9 or 10) but still waking on a high (usually 15+) to remedy this, i increased the night-time BI by 1 unit first, then another unit, which seemed to hold it better, but then i had another severe hypo, this time with a seizure, which resulted in hospitalisation.

Needless to say, after this, i was scared to go to bed on anything below 9 or 10, as this is what it crashed from before, but this almost always resulted in the super highs each morning, which wouldn't respond to corrective all day, and then plummeted in the evenings. I saw my HCP who fitted a libre FGM and altered my insulin to the below.

Morning BI (Levemir) 16u
Breakfast QA (Humalog) 1 1/2:1
Lunch QA (Humalog) 1:1
Dinner QA (Humalog) 1:1
Bed BI (Levemir) 9u

I'm now finding that the morning BG is usually high but tends to reach normal levels around lunch time, rises up again by mid afternoon/evening, but then drops low by late evening, which if treated as a hypo, takes a while to raise (even with lucozade it takes 15-20 minutes to raise by 0.2 -0.5)

I'm also on other medication for various things, not too sure if these could affect BG (apart from Metformin obviously)

Metformin 500mg 1x daily
Epilim 500mg 1x daily
Levothyroxine 300mcg

Does anyone have any ideas / tips on what i can try to get these BG's within the target range all the time?

I am awaiting my call up from GP for this years Hb1c, which hopefully will break the 8 barrier which I ve stuck on for last few years. I m hopeful.......My experience in the 1970s was much the same as yours!

For 18 years i did not keep any records of BG, And the information i received early on with regards to carbs ect was totally misguided and my only form of control was judged by eye on a daily basis. I have not even tested for Ketones for about 10-12 years.

Funny story: when I was first diagnosed,(1973)I was promised the BMA book on diabetes by my consultant: I eventually got the book 5 years ago after I purchased it from the chemists Smile Apart from this the only info I had about diabetes til DAFNE came along was dire warnings about kidney failure/eye damage, as I was nt keeping sufficient record of urine/blood tests: DAFNE was a real 'reboot' but sadly for some less talked about side effects (erectile dysfunction) too late! Embarassed

Thankyou for your response Neville, After doing the Dafne course last week i do truly believe the benefits from this concept of diabetes is going to change my life now. I do genuinely wish that when people become a diabetic - Health care professionals would explain a lot more about complications in later life as some of the issues that i have experienced could have been avoided much earlier (I don't mean to sound like i am passing the book) but in this circumstance i definitely think knowledge is power.

I was warned of eye issues from diagnosis in 1973 but managed to avoid any problems beyond short-sightedness until about 2009 when there was a bit of blood vessel growth in my left eye which required a few zaps of the laser. Prior to DAFNE, had a few sessions on my right eye (about 1 year pre DAFNE) but not enough to challenge my driving licence, although I did have to do a peripheral vision test at Swansea's behest! At my last eye appointment ( first post DAFNE) received high praise from consultant as to the state of my right eye, so it seems good control is of benefit! Its kinda important for me to have a good right eye as the left has been useless since a squint correction in the early 1960s, pre diabetes! I m possibly quite lucky with my eyes, though prior to the last lot of laser, I did find night driving a little irritating.....beyond that no problems.....

Hi, I have had a lot of problems with my eyes over the past 4-5 years due to very poor control at an early stage of my diabetes, I would like to see what other diabetics have experienced and what side effects people have experienced as i have had some serious ones.

@dunkers7 thanks for your reply.

I did consider the BI running out, but I tend to take that before bed, so at around 11.00, and also that it could be the so-called 'dawn phenomenon' - it has happened in the past, but only up by a couple of points, not double.

I'm now convinced now that its the vegetable juice. I was 6.8 this morning when I woke up, I made the juice and after 30 mins I was up at 12.9.

Juice was 3 sticks of celery, half a carrot, half a fennel bulb, a small yellow pepper, some ginger and a squeeze of lemon.

I posted the same comment on another site and someone came back to me saying that by stripping out the fiber I'm 'freeing' the sugars. I think they have a point and plan to do some research into this when I have a moment.

I'm going to move to green smoothies asap - my blender is being repaired so I'll start as soon as I get that back - and see how it goes. If this theory is correct, then by blending and keeping the fibers in the drink the impact on my sugars should be much lower.

Pity though, I thought I was doing something good for my health, but turns out it was quite the opposite .... Crying or Very sad

Do you get a rising blood glucose if you're not having the veg juice first thing?

I wonder if it really is the veg juice that's causing this, or perhaps BI starting to run out after 07:00 for example? If you try not having the veg juice one morning, you'd isolate what's going on so you can act appropriately.

If lack of BI isn't the cause, maybe you just need to take QA for the veg juice at that time in the morning when you're in a fasted state. Some folks on here have recently mentioned in other threads seeing an increase in BG if eating protein for example.

I'm like you Marke with a bag of peanuts...can't resist emptying the bag once started. They always have a drastic effect upon my BGs and I have seen BGs of 13+ after over-indulging. As a consequence, I try to avoid buying them.....Get thee behind me temptation Embarassed

I've recently started to juice veg in the morning - I read that the best time to drink this is on an empty stomach, so I'm having it before leaving home to go to work (where I have my breakfast).

I've been doing it for about a week but only started monitoring before an after reading these last couple of mornings. I realized with some shock that my sugars have shot up after taking the juice. For example, yesterday I was delighted with my 5.8 reading @ 7.00 but by the time I got to work @ 8.30 this had shot up to 11.3 and this morning from 7.2 to 15.6.

I vary the ingredients most days, but mainly I'm using celery, cucumber, fennel, kale, ginger and a drop of lemon - no fruit. So I was really surprised at the result.

I'm gong to continue for the next few days a keep monitoring closely because it seems so odd that these vegetables which would normally have zero effect suddenly become so potent once juiced.

Any one else had this experience?

As Dunkers says, it would be great if you could post your before bed readings and wake-up readings every time. Getting the basal rate right is the first step, and unless that is correct, then there is little hope for good management. Looking at all the red, I suspect that your basal is too low, but too hard to diagnose without the important bed time and wake-up readings.

The basal looks like it's holding overnight to me if 26/02 -> 27/02 is anything to go on. But it's hard to tell without more tests before bed as you say.

I reckon getting the overnight BG in a good range is the main priority here. Think about this - when you sleep you're fasting, and it accounts for about 1/3rd of a 24 hour day if you're getting 8 hours sleep. Getting that one right is the best place to start to improve the overall hba1c.

DAFNE says eat as much as you want to, so long as you're carb counting and taking bolus as appropriate. Good news if you feel stressed and could do with some comfort food. Smile

Hi,

I'm having a bit of a diabetic burnout due to stress going on in my life atm. I want to try & get back on track. I seem to be lacking with - bedtime testing / waking testing / quite often I have a late tea but then often snack on crisps or more recently popcorn halfway through the evening but with no bolus.

Any help in identifying trends or any other help ie with targets & ratios will be greatly appreciated.

Also any help in saying whether I should be reducing or increasing my basal. I've been taking 14u of Lantus basal at bedtime for quite a while now.
Really need maybe some encouragement to do some overnight basal testing.

Hi Dunkers, yes I find the same. Exercise in the morning before breakfast will see steady BGs, sometimes even higher BGs when I come back if it has been a long session and my basal has started to run out.

I find that exercise with any QA in my system will almost always lead to hypos, so I try and exercise at least 4 hours after injecting my Humalog.

Hi Thomas, without knowing you insulin regime, this is impossible to answer. It would depend on when your last injection was, what your blood glucose was at the time of that injection, what you have eaten since then and so on. Every diabetic is different, so there in no specific time that all diabetics inject insulin.

I'd suggest recording your diary using the "My Blood Glucose Diary" link to the right, then sharing the diary with the forum for advice on how to gain better control.

Its the same effect, peanuts don't contain carbs just protein. In theory that gets converted to BG so slowly that your BI will cover it, but Diabetes is full of theories that don't necessarily translate to reality. We are ALL different and react slightly different to different things like Insulin, food, illness etc etc
