Topic: General Discussion / My BGs - lots of drops here The only real disadvantage to splitting is that there is an extra injection each day.- This wouldn't really bother me because I already have x4 injections However, if you have ever forgotten to take basal insulin before, the benefit of splitting is that your BGs shouldn't rise quite so much if that happens as there will still be some in your body from the previous injection. - ![]() Splitting is reasonably easy, just drop a unit at night and add one each morning. You may find that your overall basal insulin needs will increase if your basal has been running out, so do keep an eye on that. I went from 18 units nightly, to 2 x 10 unit injections morning and night. I lost the spikes between dinner and bedtime though so I found it was worth it. - Yes all understood ![]() You may want to monitor your BGs for another week or so before you decide. If you do that, try to avoid eating anything after dinner, and just take correctional doses if required before bed. If you have good BGs before bed, then schedule a 3 am test to see if they are holding, dropping or increasing. - Maybe a good idea to monitor for another week or so. Also I'm only going to do bedtime tests at least 4/5 hours after dinnertime to give a truer reflection of bedtime tests. I sometimes eat dinner at 7:30pm - 8pm ish then bedtime test is only 2 hours after which doesn't give a true reflection of my bedtime tests does it? I do bedtime tests at 10:30pm ish because this is when I take my BI as a way of remembering to take it. |

BI - lantus and QA - Humalog I don't think that splitting your BI will help your overnight differences. Ok You may want to split your BI though if your BGs are consistently rising between dinner and bedtime as that would be a sign that your BI is running out. There is some evidence in your diary to indicate that this may be occurring. Splitting may fix that, and if that gets fixed, then it may also lead to an improvement in overnight stability. Ok but don't know whether to do a couple of nights basal testing first or do a split straight away. Splitting is absolutely fine with my practice nurse who looks after my diabetes. |

Topic: General Discussion / Lantus - Splitting Oh, and when you split, what you do is drop a unit at night, and add a unit in the morning each day until you get your 50/50 split. Don't just drop half your evening basal at night and add it in the morning or you will have wicked highs. If you are on 18 units, then it will take 9 days to get to the split, and a couple of more days after than before things stabilise so don't panic too much if you find your BGs are a little out of control over that period.So to clarify I'm on 18u at the moment then 1st night take 17u & just 1u the following morning 2nd night take 16u & 2u the following morning 3rd night take 15u & 3u the following morning 4th night 14u & 4u........ 5th night 13u & 5u....... 6th night 12u & 6u...... 7th night 11u & 7u........ 8th night 10u & 8u....... 9th night 9u & 9u ? Now could the same be done like this if you were going back onto a single dose? |

Topic: General Discussion / Lantus - Splitting I've seen a lot lately about it being the best thing people have done to split their lantus - I know we are all different. I've also read an article somewhere that lantus can be a split injection.I'm currently taking 18u of Lantus once a day at bedtime & seriously considering doing a split. I'm unsure whether to start with a straight 50/50 split 12 hours apart or do I take slightly more when I do the am dose with it being during the day & being more active then slightly less when I do the evening dose & being less active. Any help would be appreciated x |

My diabetes care is done at my GP surgery & it's the practice nuses who takes care of my diabetes. I also go to my local diabetes drop in clinic as & when I want/need to & it's a DSN who runs that. Between the practice nurse & the DSN they always seem very happy to let me make any changes on my own then visit them if I have any problems. I don't have a meter for blood ketones but I do have some urine ketone testing strips & the last one about a week ago was normal. |

JontyW Hi Jonty, Thank you for your reply & for taking a good look at my data. 1) My correction factor is 1u for 3mmol 2) The three occasions when I may not have corrected at bedtime is I'm afraid laziness :-( ie I've done 1 injection which is BI so too lazy to start having to do another injection 3) Yes I too have been noticing the massive overnight drops. I last did overnight basal testing on 01/07 @ 5:40am BG was 4.1 6:44am BG was 3.3 6:59am BG went to 5.5 after hypo treatment then on 03/07 @ 4:24am BG was 9.3 6:49am BG was 6.9 then on waking at 9:43am BG was 3.2? 4) Yes I am certain that my BI was 18u every day |

Round down if BG is around 4 - 5 ish then up if BG is 6 - 7 ish or over. |

How would you round off carbs for something which says on the packet 42.3g per pie? Also if you had a meal which was: sandwich - 36g soup - 12g total = 48g how would you round this off to the nearest 5? |
