Hi everybody

I've been on an accu-check combo pump for 2 and a half years but recently i find that i have to adjust my basal rates every other week to stop myself either being high all the time or low all the time

I make very small adjustments, usually up or down 0.01 every hour for 24 hours = 0.24 units and this seems to work for a week then it will change again

Does anyone else have to make adjustments to basal rates as regularly as this? I saw a doctor about this recently who suggested changing sites but this doesnt seem to have made things any better

If these regular adjustments are normal then i can just carry on but it seems to be a constant battle to get my basal dose correct

I will try to do a full basal test once a month.........sometimes I dont though.......

I think with the delivery of the basal being so accurate it means we are a lot more sensitive when there is a change in the livers output.......

Sometime this can be dealt with with bolus doses..........

What changes have you observed when doing your basal testing.....?

Is there much requirement for more or less........how much do you need to adjust it by?

In general though, your basal needs will change from time to time, everyone just hopes that the big changes come less often......

I am lucky if I can go a week without some sort of tinkering. I have talked with both Diabetic Educator and Endocrinologist who both say for some it is never a set and forget system but one that varies all the time. I try not to change it reflexively but rather wait until I have several days' data before tinkering. I also find using temp basals (sometimes daily) to be useful. If I get to 4.2 I go onto -10% for an hour etc. It does require monitoring. Similarly if I get to 8.5 (with no carb/IOB left) I will +10% for an hour rather than "correct". They are not big changes but they are changes. Comment was "you have been on exogenous insulin for more than 35 years it will take a toll".
