Hi Sharon, not sure what you mean by physical monitoring diary. What is it you actually want to keep a record of?

Sharon, If you mean the DAFNE Diary then your DAFNE centre will be able to provide them to you (assuming you're a graduate). Alternatively they can be bought from the DAFNE Shop (http://www.dafne.uk.com/DAFNE_Shop-64.html).
